OBJECTIVE: To investigate the clinical efficacy of FOLFOX6 and FOLFIRI in the conversion treatment of potential resectable colorectal cancer with liver metastasisMETHODS: by retrospective analysis method,according to the inclusion and exclusion criteria,collected the clinical data of the patients who were potential resectable colorectal cancer with liver metastasis received conversion chemotherapy using FOLFOX6 or FOLFIRI in our hospital from January 2010 to January 2016,comparative analysis of two groups of DCR?RR?the R0 resection rate and the 1 year survival rate.RESULTS: 1.According to the inclusion and exclusion criteria,were collected to meet the study criteria for potentially resectable medical records of 98 patients with metastatic colorectal liver,among them,male 76 cases(77.6%),22 cases were female(22.4%),? 45 years old in 75 cases(76.5%),< 45 years in 23 cases(23.5%),the average age was 52.41±12.729 years,the conversion chemotherapy using FOLFOX6 in 71 cases(72.4%),using FOLFIRI in 27 cases(27.6%),the primary lesion from colon in 38 cases(38.8%),60 cases from rectum(61.2%).2.A total of 71 cases who received conversion chemotherapy using FOLFOX6 in this study,including 53 males(74.6%)and 18 female s(25.4%);27 cases who received conversion chemotherapy using FOLFIRI in this study,including 23 males(85.2%)and 4 females(14.8%);FOLFOX6 group RR,DCR were respectively 33.8%and 96.4%;RR,DCR in FOLFIRI group were respectively 29.6% and 88.9%,two groups of Chemotherapy response rate was similar,the difference has no statistical significance(P > 0.05).3.This study there were 32 patients had received radical resection after conversion chemotherapy,including 22 cases of FOLFOX6 group and 10 cases in FOLFIRI group.the R0 removal rate between FOLFOX6 group and FOLFIRI group resection rate were respectively 31% and 37%,the difference was not statistically significant(?~2=0.326,P=0.568 > 0.05).4.The adverse reactions of conversion chemotherapy were mainly from the blood system(leukopenia,thrombocytopenia),digestive system(nausea / vomiting,diarrhea),nervous system(peripheral neuritis),the majority was I-III grade but IV adverse reactions did not appear.There was no significant difference in blood system toxicity between group FOLFOX6 and group FOLFIRI(P>0.05).The neurological toxicity caused by chemotherapy in group FOLFIRI was lighter than that in group FOLFOX6(P< 0.05).The nausea,vomiting and diarrhea in group FOLFIRI were significantly higher than those in group FOLFOX6(P<0.05).5.A total of 94 patients were followed up successfully,and 4 patients were lost.Among the missing patients,3 patients in group FOLFOX6 and 1 patient in group FOLFIRI,the follow-up rate was 95.9%(94/98).68 patients of FOLFOX6 were followed up for 1 year survival rate was 73.5%(50/68),26 patients of FOLFIRI were followed up for 1 year survival rate was 80.8%(21/26).There was no significant difference between the FOLFOX6 group and the FOLFIRI group in the 1 year survival rate(P>0.05).Survival analysis by Kaplan-Meier showed that there was no significant difference in survival time between the two groups(?~2=3.153,P=0.076 > 0.05).CONCLUSIONS: 1.Conversion chemotherapy for patients who were potentially resectable colorectal cancer with liver metastasis,could narrow the size of the liver metastasis,improve the radical resection rate,improve the prognosis in some patients,besides its adverse reactions could be tolerated,is a safe and effective transformation treatment method.2.As for the patients who were potentially resectable colorectal cancer liver metastasis received conversion chemotherapy,the efficacy of FOLFOX6 and FOLFIRI were similar.The adverse reactions of FOLFOX6 had focused on the nervous system toxicity,and the adverse reactions of FOLFIRI had focused on the digestive system as the main toxicity.
