Objective: To compare the efficacy of linear stapler against traditional manual suture in the closing of pharyngeal defect following a total laryngectomy of laryngeal carcinoma. Methods: Retrospective collected 23 cases of pharyngeal closure using linear stapler after total laryngectomy in our hospital in 2014.12-2017.03,included 17 cases of closed technique and 6 cases of open technique. At the same time, the patients were collected by manual suture pharyngeal closure after total laryngectomy in 23 cases. We compared the general data of two methods, the time of operation, the incidence of pharyngocutaneous fistula, the time of oral feeding, the length of postoperative hospital stay and the complication of postoperative except pharyngocutaneous fistula. And we explored the clinical effect of linear stapler. Results: The closed technique group of the linear stapler was able to significantly shorten the operation time by 141.12 minutes compared with the manual group;shorten the postoperative oral feeding time(4.42 days)andpostoperative to discharge time(4.75 days). The open technique group compared with the manual group can significantly reduce the operation time, but the postoperative oral feeding time, the incidence of pharyngocutaneous fistula postoperative hospital stay had no differences. There was no pharyngocutaneous fistula in the closed group, but had no significant difference with compared the control group(26.09%),P = 0.066> 0.05? The linear stapler group did not increase the incidence of complications except the pharyngocutaneous fistula. Conclusions: Closed technique with linear stapler in laryngeal neoplasms under total laryngectomy closing to pharyngeal defect is superior to traditional manual suture.
