Objective:Timely know clinical application of antibacterial drugs and bacterial drug resistance in the jurisdiction or medical institutions,regular to evaluate the clinical application of antibacterial drugs management and bacterial drug resistance forms,According to the monitoring situation,gradually establish a perfect mechanism of clinical application of antibacterial drugs can make antibiotics application more standardized and rationalization.So dynamic monitoring of bacterial distribution and Changes in drug resistance in emergency department of our hospital,can provide the basis for the selection of the first empirical broad-spectrum antibiotics in patients with acute lower respiratory tract infection,At the same time analyze the causes of bacterial resistance and Changes,to avoid multiple drug-resistant bacteria infection.Methods:Collected the lower respiratory tract infection cases from 2013.9 to 2016.12 of our hospital emergency department,and retrospectively analyzed the bacterial distribution and drug resistance of clinical isolates.Result:A total of 805 cases of lower respiratory tract infection were collected,Aged between 38 and 95 years old,Over the age of 60 elderly patients accounted for 91.62%,A total of 258 cases of bacteria and fungus were detected,The positive rate was 31.05%.Most common fungus are 106 cases,A total of 88 cases of Candida albicans,accounting for 83.02%.Tropical Candida and aspergillus are 18 cases,accouting for 16.98%.A total of 151 bacterial were isolated,The most common bacteria were Pseudomonas aeruginosa,total collect of 39 cases,28 cases of Acinetobacter baumannii,29 cases of Klebsiella pneumoniae,Other bacteria are Haemophilus parainfluenzae,Streptococcus pneumoniae,Staphylococcus aureus and so on,total collect of 54 cases.In recent years,the proportion of Pseudomonas aeruginosa decreased,the proportion of Acinetobacter baumannii increased significantly and the proportion of multi-resistant bacteria increased,almost all detected Acinetobacter baumannii is resistant to bacteria.There was no significant difference in the detection rate of Klebsiella pneumoniae in 3 years.Pseudomonas aeruginosa is highly sensitive to ?-lactamase inhibitors and aminoglycoside antibiotics,so it is recommended joint application.Not only can shorten the course of treatment,but also can reduce the occurrence of drug side effects;Pseudomonas aeruginosa resistance to carbapenem antibiotics has an increasing trend,and frequent use of carbapenem antibiotics will induce the emergence of multi-resistant strains,so it is recommended in the treatment should be used with caution,and strengthening the multi-drug resistant Pseudomonas aeruginosa strains detected,timely preventive measures.Conclusion:There is an upward trend in bacterial resistance in my department,multi-resistant bacteria is widespread.According to the bacteria spectrum and sensitive to medications to choice antibiotics,to prevent the emergence of resistant bacteria is of great significance.
