Objective:The effects of different doses of vitamin D on the bone mineral density of preterm infants were evaluated by the treatment of 105 cases of preterm infants with different doses of vitamin D for 3 months.The changes of bone mineral density(BMD)and bone metabolism-related hormone levels were measured.Methods:Collected in August 2015 to June 2016 in Kunming Medical University Second Affiliated Hospital was born and immediately transferred to the neonatal hospital in the premature children.Excluding pregnant women suffering from pregnancy complications,long history of glucocorticoid use and heart and lung,kidney,Diabetes and other diseases,premature births with congenital malformations,severe asphyxia,multiple births and other metabolic diseases,and follow-up.The final inclusion of 105 cases of premature children.According to gender is divided into male group,female group;According to birth weight divided into:<2500g group,>2500g group.According to gestational age:Group A(28 weeks to 31+6 weeks),group B(32 weeks to 33 + 6 weeks),group C(34 weeks to 36 + 6 weeks).At the beginning of one week after admission,400 IU/d and 800 IU/d vitamin D were randomly given.The bone metabolism-related hormones were measured on the day after admission.Ultrasound bone density was measured within one week after admission and at three months.The results were analyzed by SPSS statistical software.Results:1.The average bone mineral density of male group and female group was 2854.67±151.244m/s,2851.14±136.436m/s;The average bone mineral density(BMD)of premature infants in the group of<2500g group and>2500g group was 2812.16±133.821m/s,2950.32±188.196m/s;The average bone mineral density of premature infants in group A,B and C were 2701.00±91.653m/s,2807.36±138.334m/s and 2891.50±133.882m/s,respectively.There was no significant difference between male and female groups(P>0.05)?The difference between the other groups was statistically significant(P<0.05).2.The mean bone mineral density(BMD)of 400IU vitamin D group was 2786.50±9.192m/s,2883.12±157.425m/s,2941.27±115.216m/s,supplemented with 800IU vitamin D group,the mean values were 2843.166±138.420m/s,2898.00±142.225m/s,2947.06±153.185m/s.According to the statistical analysis,there was no significant difference between 400IU group and 800IU group(P>0.05).3.In sexuality the mean values of serum 25-(OH)VD in preterm infants were 22.76±10.280ng/ml and 17.04±6.504ng/ml respectively.The mean values of serum 25-(OH)VD in premature infants weight<2500g and>2500g were 18.62±7.963ng/ml?23.23±10.772ng/ml;25-(OH)VD were 16.65±6.751ng/ml,17.44±7.282ng/ml,21.52±9.753ng/ml,and the mean values of serum 25-(OH)VD in group A,B,C.The levels of serum 25-(OH)VD in group A,B and C were not significantly different(P>0.05),and the difference was significant(P<0.05).The mean values of 25-(OH)VD in the 400IU vitamin D group were 21.10±9.475ng/ml,21.05±4.928ng/ml,25.52 ± 8.338ng/ml,respectively,in the three groups of A,B and C,The mean values of serum 25-(OH)VD in the 800IU vitamin D group were 21.33±9.227ng/ml,24.91±6.873ng/ml and 27.50±10.131ng/ml,respectively.There was no significant difference between the 400IU group and the supplemented 800IU group(P>0.05).4.The average value of calcitonin in male group and female group was 11.08±8.002pg/ml,11.99 ± 6.998pg/ml;<2500g group,?2500g calcitonin average preterm children were 11.54±7.068pg/ml,11.48±8.597pg/ml;The average calcitonin in group A,B and C were 15.26±6.455pg/ml,10.14±5.309pg/ml,11.69±8.334pg/ml.There was no significant difference between the two groups(P>0.05).At the age of three months,the average calcitonin in group A,B and C were 14.67± 7.155pg/ml,11.43 ± 7.790pg/ml and 12.86±7.535pg/ml respectively.By statistic analysis,there was no significant change in the level of calcitonin in the three groups(P>0.05)5.The average value of osteocalcin in male group and female group was 87.40±31.123ng/ml?83.13±24.342ng/ml;<2500g group,?2500g osteocalcin average preterm children were 87.45±31.454ng/ml?80.24±16.416ng/ml;The average osteocalcin in group A,B and C were 15.26±6.455pg/ml,10.14±5.309pg/ml,11.69±8.334pg/ml.There was no significant difference between the two groups(P>0.05).At the age of three months,the average osteocalcin in group A,B and C were 74.92±23.533ng/ml?86.07±24.236ng/ml?84.77±23.525ng/ml respectively.By statistic analysis,there was no significant change in the level of osteocalcin in the three groups(P>0.05).6.The average value of parathyroid hormone in male group and female group was 89.36±56.244pg/ml?116.36±81.706pg/ml;<2500g group,?2500g parathyroid hormone average preterm children were 177.56±77.627pg/ml?122.02±67.289pg/ml?84.76±63.902pg/ml;The average parathyroid hormone in group A,B and C were 177.56±77.627pg/ml?122.02±67.289pg/ml?84.76±63.902pg/ml.There was significant difference between the two groups(P>0.05),and the difference was significant(P<0.05).At the age of three months,the average parathyroid hormon in group A,B and C were 123.05±28.081pg/ml?103.25±53.555pg/ml?79.56±42.619pg/ml respectively.By statistic analysis,there was no significant change in the level of parathyroid hormon in the three groups(P>0.05).Conclusion:1.The incidence of bone mineral density and serum 25-(OH)VD were lower in the premature infants,and the lower the gestational age,the lower the birth weight and the lower the bone mineral density,whereas the serum 25-(OH)VD was not related to gestational age.2.Preterm children in the first 8 days after birth to add vitamin D400IU/day or 800IU/day.There was no significant difference in bone mineral density and serum 25-(OH)VD between the three months of age.3.Preterm children osteocalcin,calcitonin and gender,gestational age,birth weight was not related.4.Parathyroid hormone was negatively correlated with gestational age.
