BackgroundChronic obstructive pulmonary disease(COPD)is a kind of chronic airway inflammation reaction disease which is common in respiratory system.In recent years,the morbidity rate of COPD is increasing year by year,the mortality and morbidity are high,seriously affect the quality of life of patients,the main clinical manifestations is lung function decreasing,dyspnea,poor body movement endurance.Eliminating airway inflammation,improving breathing difficulties and other clinical symptoms,improving lung gas exchange,delaying disease progression are the main treatment purposes.Respiratory rehabilitation training can increase the body's aerobic capacity,improve exercise tolerance,and then improve the patient's lung function and blood gas index,improve quality of life.ObjectiveTo study the clinical effects of heparin assisted budesonide and formoterol combined with respiratory rehabilitation training in treating of COPD,analyze the influence on lung function,blood gas index,coagulation function,hemorrheology parameters and inflammatory factors.Methods164 COPD patients were selected,and divided into control group(n=50),treatment group(n=55)and rehabilitation group(n=59).All patients received conventional treatment.(1)Control group: given conventional treatment only.(2)Treatment group: given budesonide and formoterol combined with low-dose low molecular heparin.(3)Rehabilitation group: given respiratory rehabilitation training on the basis of treatment group.(4)Observation and analysis index:(1)Compared the clinical efficacy of 3 groups after treatment.(2)The BODE index,mMRC and TDI score of the 3 groups were compared before and after treatment.(3)The lung function,blood gas index,coagulation function,hemorrheology parameters,inflammatory factor were compared before and after treatment.(4)Exercise endurance and quality of life of the 3 groups before and after treatment were compared.Results(1)After treatment,the total effective rate of rehabilitation group was higher than that of the treatment group,and the treatment group was higher than that of the control group,the total effective rate between the 3 groups were significantly different(P<0.01).(2)After treatment,BO DE score of the rehabilitation group was lower than that of the treatment group,and the treatment group was lower than that of the control group(P<0.01).After treatment,the mMRC score of the rehabilitation group was lower than that of the treatment group,and the treatment group was lower than that of the control group(P<0.01).After treatment,the TDI score in rehabilitation group was significantly higher than that of the treatment group,the TDI score in treatment group was significantly higher than that of the control group(P<0.01).(3)After treatment,the lung function,blood gas index,coagulation function,inflammatory factor of each group were improved,the improvement degree of the rehabilitation group was higher than that of the treatment group,and the treatment group was higher than that of the control group(P<0.01).After treatment,there were no significant changes in hemorrheology parameters in the control group(P>0.05),the levels of hemorrheology parameters were significantly decreased in the treatment group and rehabilitation group(P<0.05 or P<0.01),and the difference was significant in 3 groups(P<0.05 or P<0.01).(4)After treatment,the 6 MWD in rehabilitation group was higher than that of the treatment group,and the treatment group was higher than that of the control group(P<0.01).After treatment,the SQRG total score of the rehabilitation group was lower than that of the treatment group,and the treatment group was lower than that of the control group(P<0.01).ConclusionLow-dose low molecular heparin assisted budesonide and formoterol in the treatment of COPD,supplemented by appropriate respiratory rehabilitation training can significantly improve clinical efficiency,improve patient's symptoms such as dyspnea,reduce serum inflammatory factors of COPD patients,reduce airway inflammatory response,improve lung function,enhance respiratory muscle strength and exercise tolerance,improve the quality of life,and can also improve coagulation function of COPD patients and blood gas index,effectively prevent venous thromboembolism.
