Objectives:To investigate the value of DCE parameters in diagnosis of prostate cancer with the curve of Wash-out type,analyze the DCE time signal curve of the parameters of the value of prostate cancer diagnosis Specificity.Further screening PI-RADS V2 3-4 scores were used to evaluate the diagnostic value of PI-RADS V2.Methods:1.Patients with prostate disease who underwent MRI examination before the endocrine,radiotherapy and surgical treatment from March 2015 to August 2016 were included in the study.And 1 month after MRI examination confirmed by biopsy.Check the equipment for Philips Achieva3.0T,scanning sequence including T2WI,DWI and DCE-MRI.DCE parameter values include Relative Enhanment,Maximum Enhancement,Time To Peak,Wash out Rate,Brevity of Enhancement,Area under the curve.2.According to the pathological results into the prostate cancer group,benign prostatic hyperplasia,prostatitis disease group,the DCE values of parameters among the three groups first single factor variance analysis,a further 22 LSD-t test,P<0.05 considered statistically significant difference.3.Further screening PI-RADS V2 score of 3 patients,x2 test to calculate the DCE outflow curve of false positive,false negative.Results:1.There were 40 cases of prostate cancer,benign prostatic hyperplasia(BPH)(n = 17),and there were a total of 3 patients with prostatitis.The age ranged from 49 to 84 years,with an average age of 67 years.The serum PSA was positive in all patients,and the range of PSA was 5.06?>100 g/L.The main clinical manifestations were frequent urination and dysuria.2.Area of 2 prostate cancer group,hyperplasia and inflammatory lesions,peak signal intensity curve under the chi square test was performed to compare the results of the analysis,t were-0.032 and 0.0038,P<0.05,the difference was statistically significant.PCas Maximum Enhancement and the Area under the curve value were(1443.54 + 436.76),(407912.76.122210.914);BPHs Maximum Enhancement and the Area under the curve value were(1534.51 + 384.86),(450203.30±129722.690):Prostatitis Maximum Enhancement and the Area under the curve were(2108.74 +156.81),(592046 + 58728.390).3.The Maximum Enhancement,the area under the curve to distinguish between benign prostatic hyperplasia and prostate cancer AUC were 0.546 and 0.596,the best critical diagnostic value were 909.61,210729.50,corresponding to the sensitivity of diagnosis was 100%and 100%respectively,the specificity was 57.1%and 62%respectively,the better diagnosis was area under the curve.The Maximum Enhancement,the area under the curve in the differential diagnosis of lesions of prostate cancer and prostatitis AUC were 0.919,0.901,the best diagnostic critical value were 1920.80 and 505413,the sensitivity and specificity of the corresponding diagnosis were 100%,100%,79%and 84%,both higher diagnostic value.The Maximum Enhancement,the area under the curve in the differential diagnosis of prostatic hyperplasia and prostatitis lesions of AUC were 0.900 and 0.867,the best diagnostic critical value were 1877.95 and 513672,the sensitivity and specificity of the corresponding diagnosis were 100%,100%,80%and 85%,both higher diagnostic value.4.17 cases with PI-RADS V2 were score 3-4,including false positive(n = 3)and false negative(n = 1).The sensitivity was 86%,specificity was 70%,the false positive rate was 30%,and the false negative rate was 14%.Conclusions:1.Maximum Enhancement and the Area under the curve of prostate cancer,benign prostatic hyperplasia and prostatitis disease differs,prostatitis lesions in Maximum Enhancement and the area under the curve of PCa and BPH were significantly different(P<0.05),The average Maximum Enhancement Prostatitis disease>>BPH>PCa,the area under the curve of prostatitis disease>>BPH>PCa,suggesting that the differential contributes to prostate cancer,benign prostatic hyperplasia and prostatitis disease.2.Maximum Enhancement and the Area under the curve is more valuable in the differential diagnosis of prostate cancer and prostatitis disease,prostate hyperplasia and prostatitis diagnosis,but for differential diagnosis of prostate cancer and benign prostatic hyperplasia is of little value.3.The diagnosis of the curve of Wash-out type is helpful for prostate cancer,and participate in the PI-RADS V2 3 nodules score,but there are false positive and false negative,false positive higher than false negative rate.
