The Correlation Analysis Of The Patients' Proximal Femoral Medullary Cavity Shape And The Incidence Of Fracture Around The Prosthesis

Objective: To analyze the incidence,pathogenesis,and the types of fractures around the prosthesis in total hip arthroplasty.While further observation of the relationship between the incidence of fracture around the prosthesis and the shape of the proximal femoral medullary cavity of the patients who accepted the total hip replacement.compared the advantages and disadvantages of different kinds of prosthesis in total hip arthroplasty and further promote the treatment effects and safety in total hip arthroplasty.Methods: Use the method of retrospective analysis,collect 600 patients who accepted the total hip arthroplasty in our hospital in the past six years,including their basic information,operation data,imaging data and medical records,as well as the types of prostheses,the shapes of proximal femoral medullary cavity and the proportion of intraoperative and postoperative fractures around the prosthesis,and then,use statistical methods to analyze and observe the incidence of intraoperative and postoperative fracture around the prosthesis whose proximal femoral with different kinds of medullary cavity form.Results: 1.A total amount of 500 patients,including 340 male patients and 160 female patients,aging from 41 to 86,with an average age of 69.85 ± 8.74.According to the imaging examination of these patients,80%(400)of them with normal type,11%(55)of them with chimney type,and 9%(45)of them with champagne type.Among the 340 Male patients,80.88%(275)with normal type,8.82%(30)with champagne type,and10.29%(35)with chimney type.Among the 160 Female patients,78.12%(125)with normal type,9.37%(15)with champagne type,and 12.5%(20)with chimney type.2.In this study,the incidence of fracture around the prosthesis was 4%,including 10 women patients and10 male patients.Male with an average age of 75.36 ± 3.51,female with an average age was 70.23 ±5.42.In this 20 patients,champagne type 8 cases(male3?female 5),chimney type 8 cases(male5?female3),normal type 4 cases(male2?female2).The incidence of eachtypes of the fracture is 1%(normal type),14.55%(chimney type),17.78%(champagne type).The incidence of female patients is significantly higher than male,and the incidence of the patients who with champagne type and chimney type is significantly higher than normal types,the differences are both statistically significant(P < 0.05).Conclusion: In the total hip arthroplasty,the incidence of fracture around the prosthesis of female patients is obviously higher than that of male patients.By the same time,patients with champagne types and the chimney types have an obviously higher incidence of fracture around the prosthesis than patients with normal types.During the total hip arthroplasty,pay more attention to patients' gender,proximal femoral medullary cavity shape,and select an appropriate prosthesis,as well as targeted treatment,avoid the fracture around the prosthesis further more serious impact on the patients.
