Comparison Of The Clinical Effects Of Button Plate And Clavicular Hook Plate In The Treatment Of Acromioclavicular Joint Disloc Ation

Objective: To explore the clinical advantages of the button plate and clavicular hook plate on the treatment of Rockwood type ? acromioclavicular joint dislocation,the biomechanical characteristics and complications of different types of fixation for dislocation of acromioclavicular joint,providing a theoretical basis for clinical treatment and promotion.Methods:Retrospective analysis 85 patients with Rockwood ? acromioclavicular joint dislocation and other sever types during from October 2014 to October 2016,all the patients were simple dislocation without shoulder joint and clavicle fracture,and they had received surgical treatment.43 cases had received clavicular hook plate in the treatment(group A),42 cases had received button plate in the treatment(group B).The two groups were observed and recorded in detail the operation time,incision length,amount of bleeding,hospitalization time,return to work,the two operation,the VAS pain score,shoulder joint function,adverse reactions(impingement syndrome,nerve and vascular injury,joint stiffness,infection,loosening,dislocation,arthritis)and so on.Finally,Constant score were used to compare the function of shoulder joint before the treatment and one month,six months and one year after the operation and giving the evaluation of curative effect.Results: 85 patients were followed up for 12~26 months and the mean time of the follow-up was 15.5 months.There was no statistical difference in age,gender,VAS and the classification of Rockwood in both two groups(P>0.05).There is statistical significance in the operation time,the amount of bleeding,the time of hospitalization,the length of incision,return to work,the second operation,the VAS pain score and the function of shoulder joint(P<0.05).Group A occurrence of subacromial impingement syndrome and the disorder of shoulder joint movement after operation occurred highly.Conclusions: Compared with the clavicular hook plate,the button plate and clavicular hook plate has obvious advantage in the treatment of Rockwood type ? acromioclavicular joint dislocationthere and other severe types.
