The Effect Of Treatment Homogeneity Among Primary Hospitals On Reducing Ischemic Time And Improving Prognosis Of Patients With Acute Myocardial Infarction

Objective:To evaluate the effect of treatment homogeneity among primary hospitals on reducing ischemic time and improving prognosis of patients with acute myocardial infarction.Method:Training the doctors of primary hospital in accordance with uniform standards to make them can quickly diagnose and treat patients with acute myocardial infarction in accordance with the chest pain center standard process.The patients with myocardial infarction who received direct PCI before the establishment of the chest pain center as group A,The patients with myocardial infarction who received direct PCI after the establishment of the chest pain center as group B.These indicators including FMC-Confirmed time,D-to-B time,left ventricular ejection fraction,CCU hospitalization time,hospitalization rate of heart failure during one year,hospital mortality were compared.Results:According to the standard of Chinese primary chest pain center established Zhenjiang City First People's Hospital Chest Pain Center,and through the Chinese chest pain center certification,to achieve the primary hospital for patients with acute myocardial infarction homogenization.After the establishment of CPC,the first medical contact time was significantly shortened[(31.6±16.8)min in group A vs.(56.6±28.4)min in group B,P<0.01 ],the annual mean D-to-B time was also significantly shortened[(154.9±61.1)min in group A vs.(94.7±28.8)min in group B,P<0.01 ],The in-hospital mortality rate tended to be lower and the heart failure and hospitalization rates was significantly reduced in group B compared with group A[1.3 %(1/74)vs4 %(2/50),P>0.05;5.4 %(4/74)vs12%(6/50),P>0.05].The length of CCU stay was slightly shortened from(4.02±2.50)days to(3.81±1.48)days(P>0.05).Conclusion:Primary hospital homogenization diagnosis and treatment can shorten the time of acute acute infarction in patients with ischemic time and have a tendency to improve the prognosis.
