Abstract/Summary:
Objective The study aimed to investigate the prevalence,morbidity of bilirubin encephalopathy(BE)in the neonatal unit of the Guangdong Province,China.Subjects and Methods1.Subjects: According to the division of economic development in Guangdong province which include eastern,northern,western Guangdong and Pearl River Delta.In the 4 regions,we selected for several medical Institutions which have the ability to carry out neonatal care in order to carry on epidemiology survey on the prevalence and associated risk factors of BE.The medical Institutions and the infants who born in and were diagnosed BE in these medical Institutions were the subjects of the study.The diagnostic criteria for BE were referenced by local medical institutions.2.Methods: This is a retrospective,descriptive review.Through the literature research,we designed two questionnaires,before investigationg we Contact the director of the department and hold a traning.Survey conduceted in two parts,one was investigate the capacity of diagnosis and treatment BE as well as Follow-up visits in the medical Institutions,the other was collected the medical records of of all newborns,diagnosed with BE.All of the epidemiological questionnaires were collected and checked.Then they were coded and filed.At last,the questionnaires and their codes were input and checked by the professional person twice so that the data could correctly input.3.Statistical Analysis: The version 3.1 of Epidata system was used for data entry and the data were analyzed using SPSS 23.0 software package.Means and standard errors were calculated for measurement data.And prevalence rate and constituent ratio were calculated for numeration data.Analysis of variance or nonparametric statistics were used to compare statistical differences of variables.Chis-square test or fisher’s exact test was used to compare statistical differences of prevalence rates.2-tailed P value less than 0.05 was considered statistically significant.Results1.The mobility of BE in guangdong province was estimated at 0.19‰ and the prevalence of BE in this study was estimated at 3.01‰ in all neonatal admissions.In different region,levels there are no different in the prevalence of BE.2.There is no statistically significant difference of equipment in different levels and regional medical institutions.However,there are statistical differences in the follow-up requirements and the monitoring of bilirubin in the hospital during the discharge of the newborn.further standardization should be conduncted in follow-up and monitoring.3.The majority of cases were male,spontaneous labor,full-term,no PROM,and breast-feeding.The date of diagnosis of bilirubin encephalopathy was 3 days to 11 days,average ages 7 days.4.Our study presence that G6 PD and neonatal infection are the most Common complication of bilirubin encephalopathy in guangdong province.The secondery complication was hemolytic disease.5.There have no significant relationship between the complications and the TSB,B/A value.6.The infants whoes Nuclear magnetic resonance(NMR)is positive showed high risk factor for neonatal infection,and be in low levels and lower bilirubin bilirubin/albumin ratio and diagnosed later than those negtive.Conclusions The incidence of neonatal bilirubin encephalopathy is still high in guangdong province,we need to further standardize the jaundice follow-up monitoring and management,the existing infection factors of children with severe high blood bilirubin should intervene timely.
