ObjectivesThrough magnetic resonance spectrum to detect premature full-term metabolite levels in the brain,discussed:(1)could the brain metabolites predict development of the nervous system;(2)how many kinds of metabolites in the brain can predict nervous system development;(3)what kind of metabolite in the brain predict the nervous system development is the best;(4)which regional brain metabolite predict the nervous system development is the best;(5)when is the best time to predict Methods1.When the group of premature infants correcting full-term conduct MRI scan and MRS scan.2.The group of premature infants in correcting June and in correcting 1 year old accept AIMS assessment and GESELL assessment.Results1.Classification of the variables in the general information(gender,embryo number,preterm premature rupture of membranes,prenatal hormone use,late-onset sepsis,intraventricular hemorrhage,periventricular leukomalacia,mother’s occupation,mother’s education background,patent ductus arteriosus,postpartum steroid used,auditory brainstem response,test-tube baby,degree of asphyxia,neonatal hypoxic ischemic encephalopathy,hyperbilirubinemia,intrauterine infection,pregnancy-induced hypertension,dystocia,threatened abortion)with the outcome variables(AIMS scores in correcting June,development business;AIMS scores in correcting 1 year old,developmental quotient)conduct the Pearson correlation analysis found that,periventricular leukomalacia is negatively correlated with theAIMS scores in correcting June(P < 0.05),periventricular leukomalacia the higher,the AIMS scores correcting in June is lower;Periventricular leukomalacia is negatively correlated with development quotient in correcting 1 year old(P < 0.05),that is to say,periventricular leukomalacia the higher,the developmental quotient in correcting 1 year old is lower;Degree of asphyxia is negatively correlated with the AIMS scores and developmental quotientin in correcting 1 year old(P < 0.05),that is to say,degree of asphyxia the higher,AIMS scores and developmental quotientin in correcting 1 year old lower;Degree of asphyxia is negatively correlated with the social behaviour in correcting June(P < 0.05),that is to say,the degree of asphyxia higher,the score social behaviour in correcting June lower.Degree of asphyxia is negatively correlated with gross motor skills and social behavior in correcting 1years old(P < 0.05),that is to say the degree of asphyxia higher,the gross motor skills and social behavior score in correcting 1 years old lower.2.General information of the continuous variables(gestational age,birth weight,and 1 minute Apgar score,5 minute Apgar score,assisted ventilation times,oxygen cure times,hospitalization days,circumference,length)respectively with the outcome variables(AIMS scores in correcting June,development business;AIMS scores in correcting 1 year old,developmental quotient)conduct the Pearson correlation analysis found that,assisted ventilation times is negatively correlated with AIMS scores in correcting 1 year old(P < 0.05),that is to say the assisted ventilation times is longer,the AIMS scores scores in correcting 1 year old is lower.Gestational age is positively correlated with the gross motor skills in correcting June(P < 0.05),that is to say the higher gestational,the higher score of gross motor skills in correcting June.Gestational age is positively correlated with adaptability in correcting 1 year old(P < 0.05),that is to say,the higher gestational age,the higher score of adaptability in correcting 1 year old.3.Classification of the variables in the general information(birth weight,the degree of asphyxia,neonatal hypoxic ischemic encephalopathy,hyperbilirubinemia,intracranial hemorrhage,intrauterine infection,pih,dystocia,threatened abortion)with the outcome variables(cerebral palsy)single factor chi-square analysis foundthat,there was no statistically significant difference,(P > 0.05).4.Frontal metabolites(NAA,Cho,Cr,MI,Lac,NAA/Cr,Cho/Cr,MI/Cr,Lac/Cr)respectively with the outcome variables(AIMS scores in correcting June,development business;AIMS scores in correcting 1 year old,developmental quotient)conduct the Pearson correlation analysis found that,there were no statistically significant difference,(P > 0.05).5.Basal ganglia metabolite(NAA,Cho,Cr,MI,Lac,NAA/Cr,Cho/Cr,MI/Cr,Lac/Cr)respectively with the outcome variables(AIMS scores in correcting June,development business;AIMS scores in correcting 1 year old,developmental quotient)conduct the Pearson correlation analysis found that,the basal ganglia MI is negatively correlated with development quotient in correcting 1 year old(P < 0.05),that is to say,the basal ganglia MI is higher,the development quotient in correcting 1year old is lower;Basal ganglia MI is negatively correlated with gross motor skills,fine motor skills,adaptability,social behavior in correcting 1 year old(P <0.05),that is to say,the basal ganglia MI is higher,the gross motor skills,fine motor skills,adaptability,social behavior score in correcting 1 year old is lower;Basal ganglia Cho/Cr is negatively correlated with fine motor skills in correcting 1 year old(P < 0.05),that is to say,the basal ganglia Cho/Cr is higher,fine motor skills score in correcting 1 year old is lower;basal ganglia MI/Cr is negatively correlated fine motor skills,adaptability,language,social behavior in correcting 1 year old(P <0.05),that is to say the basal ganglia MI/Cr is higher,the fine motor skills,adaptability,language,social behavior score in correcting 1 year old is lower.6.Hippocampal metabolites(NAA,Cho,Cr,MI,Lac,NAA/Cr,Cho/Cr,MI/Cr,Lac/Cr)respectively with the outcome variables(AIMS scores in correcting June,development business;AIMS scores in correcting 1 year old,developmental quotient)conduct the Pearson correlation analysis found that,the hippocampus Lac/Cr is negatively correlated with developmental quotient in correcting June(P < 0.05),that is to say,the hippocampus Lac/Cr is higher,the lower of developmental quotient in correcting June;The hippocampus NAA/Cr is positively correlated with gross motor skills in correcting June(P < 0.05),that is to say,the hippocampus NAA/Cr is higher,the gross motor skills score in correcting June is higher;The hippocampus MI is negatively correlated with adaptive in correcting 1 year old(P < 0.05),that is to say,the hippocampus MI is higher,lower of the adaptive scores in correcting 1 year old.7.Periventricular metabolites(NAA,Cho,Cr,MI,Lac,NAA/Cr,Cho/Cr,MI/Cr,Lac/Cr)respectively with the outcome variables(AIMS scores in correcting June,development business;AIMS scores in correcting 1 year old,developmental quotient)conduct the Pearson correlation analysis found that,Periventricular Lac is negatively correlated with development quotient in correcting 1 year old(P < 0.05),that is to say the periventricular Lac higher,developmental quotient in correcting 1 year old is lower;Periventricular Lac/Cr with AIMS scores in correcting 1 year old,developmental quotient in correcting 1 year old is negatively correlated(P < 0.05),that is to say the periventricular Lac/Cr is higher,AIMS scores in correcting 1 year old,developmental quotient in correcting 1 year old is lower;Periventricular Lac is negatively correlated with fine motor skills and gross motor skills in correcting 1year old(P < 0.05),that is to say the periventricular Lac is higher,the fine motor skills and gross motor skills in correcting 1 year old is lower;periventricular MI/Cr is negatively correlated with the gross motor skills in correcting 1 year old(P < 0.05),that is to say the periventricular MI/Cr is higher,the gross motor skills score in correcting 1 year old is lower;Periventricular Lac/Cr is negatively correlated with the gross motor skills and the fine motor skills in correcting 1 year old,(P < 0.05),that is to say the periventricular Lac/Cr is higher,the gross motor skills and fine motor skills score in correcting 1 year old is lower.8.Cerebellum metabolites(NAA,Cho,Cr,MI,Lac,NAA/Cr,Cho/Cr,MI/Cr,Lac/Cr)with outcome variables(AIMS scores in correcting June,development business;AIMS scores in correcting 1 year old,developmental quotient)conduct the Pearson correlation analysis found that,the cerebellum Lac is negatively correlated with development quotient in correcting 1 year old(P < 0.05),that is to say the higher the concentration of the cerebellum Lac,the development quotient in correcting 1 year old is lower;Cerebellum Lac/Cr is negatively correlated with the development quotient in correcting 1 year old(P < 0.05),that is to say thecerebellum Lac/Cr is higher,the development quotient in correcting 1 year old is lower;Cerebellum Lac/Cr is negatively correlated with the gross motor skill in correcting 1 year old(P < 0.05),that is to say the cerebellum Lac/Cr is higher,the gross motor skill in correcting 1 year old is lower.9.Multi-factor linear regression analysis: single factor(general information,brain metabolites)with outcome variables(AIMS scores in correcting June,development business;AIMS scores in correcting 1 year old,developmental quotient)conduct the Pearson correlation analysis,the analysis was statistically significant variables as independent variables,the outcome variables(AIMS scores in correcting June,development business;AIMS scores in correcting 1 year old,developmental quotient)as the dependent variable establish multi-factor linear regression analysis,and found that premature babies’ developmental quotient and fine motor in correcting June,the influence factors is the hippocampus Lac/Cr;The influence factors of premature AIMS scores in correcting 1 year old is the asphyxia degree and assisted ventilation’s time;The influence factors of developmental quotient in correcting 1 year old is basal ganglia’s MI/Cr,periventricular Lac,cerebellum Lac/Cr;Premature’s gross motor skills,fine motor skills,adaptability,language,social behavior in the correcting 1year old,the influence factors is basal ganglia MI,basal ganglia MI/Cr,basal ganglia MI/Cr,basal ganglia MI/Cr,basal ganglia MI/Cr.Conclusions1.Premature infant’s brain metabolite levels help predict when the full term of Premature infant’s nervous system development.2.Brain metabolite MI,Lac,NAA/Cr,Cho/Cr,MI/Cr,Lac/Cr help predict nervous system development.3.Brain metabolite MI/Cr to predict the nervous system development is the best.4.In the five areas of interest region(frontal lobe,basal ganglia,hippocampus,periventricular,cerebellum),basal ganglia is the best region to predict the nervous system development.5.Correction in June and correction in 1 year old,maybe correction in 1 year old is the best time to predicting the nervous system development.
