Efficacy Of Radical Total Gastrectomy Versus Radical Proximal Gastrectomy For Gastric Cardiac Cancer: A Meta-analysis

Objective To evaluate the effectiveness of radical total gastrectomy for gastric cardiac cancer.Methods We searched PubMed,Web of Science,The Cochrane Library,Chinese Biomedical Literature Database,Wanfang Database and China National Knowledge database to search relevant literature about radical total gastrectomy for gastric cardiac cancer,were searched from inception to 10 2016 18.According to the inclusion and exclusion criteria,the 2 reviewers independently screened the literature,extracted data and evaluated the quality,RevMan 5.3 software was used for Meta analysis.Results A total of 8 studies were included in the study,and 522 patients were included.The Meta analysis results showed that: compared with the radical proximal gastrectomy,radical total gastrectomy for gastric cardiac cancer survival rate at 5 years[OR=1.84,95%CI(1.16,2.92),P=0.01],the 3 year survival rate [OR=1.95,95%CI(1.30,2.93),P=0.001],the incidence of anastomotic fistula[OR=0.39,95%CI(0.16,0.94),P=0.03],the incidence of reflux [OR=0.12,95%CI(0.04,0.36),P=0.0002],the differences were statistically significant.No significant difference in the In the 1 year survival rate of [OR=1.31,95%CI(0.66,2.60),P=0.44],incidence of anastomotic stenosis[OR=0.53,95%CI(0.16,1.69),P=0.28],the incidence of intestinal obstruction,[OR=0.39,95%CI(0.14,1.09),P=0.07],the incidence of wound infection[OR=0.79,95%CI(0.19,3.33)P=0.75].Conclusion The current evidence shows that the radical total gastrectomy for gastric cardiac cancer with good effect and fewer complications.In view of the limited amount of research,the conclusion is still need to carry out more research to verify.
