Objective: To investigate the association of endothelial protein C receptor(EPCR)gene polymorphisms(rs2069948C/T,rs867186A/G)with the occurrence and development of sepsis,and its relation with the plasma soluble endothelial protein C receptor(sEPCR)level and the severity of sepsis in sepsis patients,screening the risk genotypes and alleles of EPCR gene in sepsis patients,providing new ideas for early diagnosis and prognosis of sepsis.Methods :Snapshot SNP genotyping assays and DNA sequencing methods were used to detect the polymorphisms of EPCR gene,rs2069948C/T(2532C/T)and rs867186A/G(6936A/G),in 64 patients with sepsis and in 113 controls,to determine the distribution of genotypes and alleles of EPCR gene SNPs in patients with sepsis and controls;Plasma sEPCR concentrations were measured by enzyme-linked immunosorbent assay(ELISA)in all subjects,to explore the relationship among EPCR gene and plasma sEPCR levels and sepsis.Results: There were no significant differences in gender and age composition between the sepsis group and the control group(P>0.05).Compared with Europeans,Japanese and Africans,the genotype and allele frequencies of EPCR rs2069948 C/T of healthy subjects in Guangxi were significantly different(P<0.05),but there were no significant difference when compared with Chinese people in Beijing(P>0.05).Compared with Chinese people in Beijing,the genotype and allele frequencies of EPCR rs867186 A/G were significantly different(P<0.05),but there were no significant difference when compared with Japanese,Africans and Europeans(P>0.05).There weresignificant differences in the allele frequencies of EPCR gene rs2069948C/T and rs867186A/G between male and female in patients(P<0.05).Females carrying rs2069948C/T genotype,T allele and males carrying rs867186 A allele were associated with a significantly increased risk of sepsis(OR=2.740,95%CI: 1.065~7.050,P=0.034;OR=2.790,95%CI: 1.358~5.730,P=0.005;OR=1.735,95% CI: 1.063 ~2.833,P=0.027,respectively).The severity of sepsis was not related to EPCR rs2069948C/T,rs867186A/G polymorphisms(P>0.05).Plasma sEPCR levels of sepsis patients were slightly higher than in control subjects and showed no correlation with EPCR gene polymorphisms.Conclusion: The frequency of EPCR gene polymorphisms in Guangxi healthy population were different from that in different ethnic groups.EPCR gene polymorphisms may be associated with increased risk of sepsis,and were sex-related,female with the rs2069948 C/T genotype,T allele and male with the rs867186 A allele had a higher risk of sepsis,but this had no effect on the release of sEPCR in patients with sepsis.
