To Detect The Relationship Between Expression Of Stat3 As Well As Survivin In Rectal Cancer Tissue And Effect Of Neoadjuvant Chemoradiotherapy

Objective: To detect the relationship between expression of Stat3 as well as Survivin in rectal cancer(RC)tissue and effect of neoadjuvant chemoradiotherapy(NCRT),explore its relationship with NCRT effect,and predict effect of neoadjuvant chemoradiotherapy and relevant sensitive molecular protein;provide theoretical reference for preparing suitable individualized therapeutic schedule for RC patients so as to offer clinical guidance to apply NCRT to RC patients.Methods: 74 RC patients received NCRT before operation and operative treatment disgnosed in Shanxi Provincial People's Hospital during January 2012-December 2014 were selected;93 cases of RC patients received operative treatment directly were screened as control group.10 cases over 10 cm away from margin of RC tissue diagnosed as normal rectal tissue by pathology;this research adopted immunohistochemical SP method and Western Blot to test the expressions of RC tissue in direct operation group,RC tissue in NCRT group and Stat3 as well as Survivin in para-carcinoma tissue;analyzed the relationship between expressions of Stat3 as well as Survivin in RC tissue of NCRT group,and clinicopathologic data.Results: 1.Western Blot indicated that,the relative molecular weights of RC tissue in direct operation group and RC tissue in NCRT group were about the specific bans of 92 KB and 16KB;but no obvious specific band was found after the detection of RC cancer para-carcinoma tissue.The gray value detection by Image J image-text analysis system showed that,the gray values of Stat3 and Survivin in RC tissue of NCRT group separately were 13611.721±1284.502 and 12930.326±1068.519;the gray values of Stat3 and Survivin in RC cancer para-carcinoma tissue separately were 6738.147 ±618.405 and 6187.325±691.437;the gray values of Stat3 and Survivin in RC tissue of direct operation group and NCRT group were all higher than that in para-carcinoma tissue,and the difference was statistically significant(P<0.01).The difference between gray values of Stat3 and Survivin in RC tissue of direct operation group,and that in RC tissue of NCRT group was not statistically significant(t=1.892,P=0.061;t=1.969,P=0.051).2.Immunohistochemistry indicated that,STAT3 positive expression was mainly located at the cytoplasm of RC histocyte while a little was located at cell nucleus,which mostly were particles in color ranging from faint yellow to deep yellow and the monitory presented dark sepia.The positive expression rates of STAT3 in para-carcinoma tissue,RC tissue of direct operation group and RC tissue of NCRT group separately were 10%(1/10),75.2%(70/93)and 65%(48/74).The positive expression rates of STAT3 in RC tissue of direct operation group and RC tissue of NCRT group were obviously higher than that in para-carcinoma tissue,and the difference was statistically significant(P<0.05).The positive expression rate of Stat3 in RC tissue of NCRT groupwas lower than that in RC tissue of direct operation group,and the difference was not statistically significant(P>0.05);Survivin positive expression was mainly located at the cytoplasm of RC histocyte which mostly were particles in color ranging from faint yellow to deep yellow;its expression in para-carcinoma cell was negative or weak positive.The positive expression rates of Survivin in para-carcinoma tissue,RC tissue of direct operation group and RC tissue of NCRT group separately were 10%(1/10),81.7%(76/93)and 67.6%(50/74).The positive expression rates of Survivin in RC tissue of direct operation group and RC tissue of NCRT group were obviously higher than that in para-carcinoma tissue,and the difference was statistically significant(P<0.05).The positive expression rate of Survivin in RC tissue of NCRT groupwas lower than that in RC tissue of direct operation group,and the difference was not statistically significant(P>0.05).3.In effective pathology relief group(TRG2+3+4)of NCRT plan,the positive expression rates of STAT3 and Survivin separately were 55.1%(27/49)and 57.1%(28/49);in ineffective pathology relief group(TRG0+1),the positive expression rates of STAT3 separately were 84.0(21/25)and 88.0%(22/25);the positive expression rates of STAT3 and Survivin were obviously lower than that of tumor progression group(PD),the difference was statistically significant(P<0.05).STAT3 and Survivinin positive chemoradiotherapy effective rates in RC tissue of NCRT group separately were 56.3%(27/48)and 56.0%(28/50);STAT3 and Survivinin negative chemoradiotherapy effective rates in RC tissue of NCRT group separately were 84.6%(22/26)and 87.5%(21/24),which indicated that,STAT3 and Survivinin positive chemoradiotherapy effective rates in RC tissue of NCRT group were higher than that in negative group.4.STAT3 and Survivinin expressions in RC tissue of direction operation group and NCRT group were not related to age,gender,yp N staging or BMI,the difference was not statistically significant(P > 0.05).However,the expressions were related to tumor differentiated degree and T staging,the difference was statistically significant(P<0.05).5.Spearman rank correlation analysis indicated that,in RC of NCRT group,Stat3 expression presented obvious positive correlation with Survivin expression(r=0.493 P=0.000).Conclusions:1.Neoadjuvant chemoradiotherapy can reduce tumor staging,increase resection rate,and improve the quality of life of patients.2.Stat3 and Survivin can be used as one of the reference indexes of sensitivity of neoadjuvant chemoradiotherapy for rectal cancer.3.The combined detection of the expression of Stat3 and Survivin in rectal cancer may reflect the biological and pathological features of rectal cancer.
