Objective To investigate the different positive rate of bone scintigraphy between vertebral metastases and MM(multiple myeloma),and its value in the diagnosis of these two disease.Methods Images of bone scintigraphy and CT in 45 patients with vertebral metastases and 27 patients with MM were retrospectively analyzed,bone destruction showed by CT were observed and recorded.These detected vertebral bodies were divided into 3groups,small(?9),mediate(10-18)and large(>18).The comparison of the different positive rate use X2 test and Fisher exact probabilities.when p< 0.05,the difference was considered to be statistically significant.Results In 45 cases of vertebral metastases,CT showed 158 vertebra were detected,of which,92 vertebra were detected by bone scintigraphy.In 27 cases of MM,165 vertebra were detected by CT,of which,54 vertebra were detected by bone scintigraphy.The positive rate of bone scintigraphy for vertebral metastases was 58.23%(92/158),it was higher than that in MM(multiple myeloma)32.73%(54/165)(?~2=21.189,p=0.000).In 45 cases of vertebral metastases,92 vertebral bodies were detected by CT,of which,47 vertebral bodies were detected by bone scintigraphy.In 27 cases of MM,95 vertebral bodies were detected by CT,of which,28 vertebral bodies were detected by bone scintigraphy.The positive rate of bone scintigraphy for the vertebral body invaded in vertebral metastases was 51.09%(47/92),it was higher than that in MM29.47%(28/95)(?~2=9.089,p=0.003).The positive rate of bone scintigraphy for large lesions of vertebral bodies in vertebral metastases was 77.42%(24/31),it was higher than that in MM 50.00%(16/32)(?~2=5.107,p=0.024).The positive rate of bone scintigraphy for mediate lesions of vertebral bodies in vertebral metastases was 41.03%(16/39),it was higher than that in MM 20.93%(9/43)(?~2=3.897,p=0.048).The positive rate of bone scintigraphy for small lesions of vertebral bodies in vertebral metastases was31.82%(7/22),it was 15.00%(3/20)in MM(?~2=0.838,p=0.360).In 45 cases of vertebral metastases,61 vertebral bodies and attachments were detected by CT,of which,44 vertebral bodies and attachments were detected by bone scintigraphy.In 27 cases of MM,67 vertebral bodies and attachments were detected by CT,of which,26 vertebral bodies and attachments were detected by bone scintigraphy.The positive rate of bone scintigraphy for the vertebral bodies and attachments invaded in vertebral metastasis was72.13%(44/61),it was higher than that in MM 38.81%(26/67)(?~2=14.310,p=0.000).Conclusion By comparing positive rate of bone scintigraphy between vertebral osteolytic metastases and vertebral Multiple myeloma,can improve our understanding and imaging diagnosis of osteolytic metastases and vertebral multiple myeloma.
