The Relevant Research Of The Expression Of PD-1/PD-L1 MRNA And EGFR Gene Mutations In Non-small Cell Lung Cancer

Abstract/Summary:
Objectives To investigate relationship between epidermal growth factor receptor(EGFR)gene mutations with the expression of PD-1/PD-L1 m RNA and the clinical significance in non-small-cell lung cancer(NSCLC).Methods There were 64 cases NSCLC and 20 cases benign lesions of lung tissue paraffin wax been collected.The mutations of EGFR gene were detected by Fluorescence PCR Capillary Electrophoresis in 64 cases NSCLC.And the expression of PD-1/PD-L1 m RNA were detected by RNAscope in 64 cases NSCLC and 20 cases benign lesions of lung tissue.Results The mutation rate of the EGFR gene was 42.19%(27/64)in 64 cases NSCLC.The mutation rate of exon 18,19,20 and 21 were 4.69%,20.31%,9.38% and 14.06%.There were 4(6.25%,4/64)double-mutation,they were T790M&G719A,T790M&E746-A750 del,T790M&L858R and S768I&L858R.The statistical anslysis results show that the mutation rate of EGFR was correlationship with patients’ sex,smoking history,histological type and differentiation degree in 64 cases NSCLC.The EGFR gene mutation rate of female patients(63.6%,14/22)was higher than male patients’(31.0%,13/42)(P=0.012).The EGFR gene mutation rate of never-smoking patients(64.3%,18/28)was significantly higher than smoking patients’(25.0%,9/36)(P=0.002).The EGFR gene mutation rate of adenocarcinoma patients(66.7%,24/36)was higher than squamous carcinoma patients’(10.7%,3/28)(P=0.000).And,the EGFR gene mutation rate of well differentiation patients(69.6%,16/23)was significantly higher than moderated and poor differentiation patients’(26.8%,11/41)(P=0.001).But,there are no significant correlation among the mutation rate of EGFR gene with patients’ age,lymph node metastasis and clinical TNM stage(P>0.05).There was significant correlation among the mutation rate of exon 19 with patients’ sex,smoking history and histological type(P<0.05).The mutation rate of 19 exon was higher in sex,never-smoking and adenocarcinoma patients.And the mutation rate of exon 21 was closely related to the differentiation degree(P<0.05).The mutation rate of 21 exon was higher in well differentiation patients.The positive expression of PD-1 m RNAwas no relationship with patients’ gander,age,smoking history,histological type,differentiation degree,lymph node metastasis and clinical TNM stage in 64 cases NSCLC(P>0.05).And,there was no correlation between the expression of PD-1 m RNA with the clinical pathological features of adenocarcinoma or squamous carcinoma(P>0.05).The positive expression of PD-L1 m RNA in younger than 60 year-old patients(21.4%,6/28)was lower than the older than 60 year-old patients’(47.2%,17/36)in 64 cases NSCLC..There are statistical difference(P=0.033).And,there was no correlation among the expression of PD-L1 m RNA with patients’ sex,smoking history,histological type,differentiation lymph node metastasis and clinical TNM stage(P>0.05).And,the positive expression of PD-L1 m RNA of well differentiation patients(25.0%,5/20)was lower than the moderated and poor differentiation patients’(62.5%)in 36 cases adenocarcinoma patients.The statistical analysis show that there are statistical difference of the positive expression of PD-L1 m RNA between well differentiation and the other differentiation degree patients(P=0.023).And,the positive expression of PD-L1 Mrna of I-II stage patients(11.1%2/18)was lower than the III-IV stage patients’(80.0%,8/10)in 28 cases squamous carcinoma patients.The statistical analysis show that there are statistical difference of the positive expression of PD-L1 m RNA between I-II stage patients and III-IV stage patients(P=0.011).The mutation of EGFR gene was no related to the expression of PD-1 m RNA in 64 cases NSCLC(r=-1.91,P=0.126).But,the mutation of EGFR gene is poditively related to the expression of PD-L1 m RNA in 64 cases NSCLC(r=0.283,P<0.05).Conclusions 1 The mutation rate of EGFR was ever highe.The primarily mutations were 19 and 21 exon mutations.The form of mutation in 18 exon was G719 A.And,The forms of mutations of 19 exon were E746_A750del,E746_A750del,E746_T751>A and L747_P753>S.The forms of mutations of 20 exon was V769_D770 ins ASV,T790 M and S768 I.The forms of mutations in 21 exon was L858 R and L861 Q.There were 4 cases doublemutation.they were T790M&G719A,T790 M &E746_A750de,T790M&L858R and S768 I &L858R.2 The mutation rate of EGFR was higher in wemen,lung adenocarcinoma,smoking and well differentiation patients.The mutation rate of 19 exon was higher in wemen,smoking and lung adenocarcinoma patients.And,the mutation rate of 21 exon was higher in well differentiation patients.3 The expression of PD-1/PD-L1 m RNA was higher than benign lung tissue.It show that the expression of PD-1 m RNA was relationshiop with the development of NSCLC.4 The expression of PD-1 m RNA was no correlation with the clinical pathological characteristics of NSCLC.The expression of PD-L1 m RNA was related to patient’s age in NSCLC.And,PD-L1 m RNA overexpresses in over 60 years of age patients.5 There were correlation between the expression of PD-L1 m RNA and degree of differentiation in adenocarcinoma patients.PD-L1 m RNA overexpresses in poor and moderately differentiation adenocarcinoma patients.The expression of PD-L1 m RNA was related to the clinical stage in squamous carcinomar.And,PD-L1 m RNA overexpresses in III-IV Stage squamous carcinoma patients.The expression of PD-1 m RNA was no correlation with the clinical pathological characteristics of lung adenocarcinoma and squamous carcinoma patients.6 The overexpression of PD-1 m RNA was no relationship with EGFR mutation.But,the overexpression of PD-L1 m RNA was positively relationship with EGFR
