Objective: Diabetes is a group of metabolic diseases characterized by chronically increased blood glucose.Recently China has a population of more than 9.2 million diabetes patients.People above 18 years old ha ve a morbidity of 9.7%.Islet beta cell function failure and insulin resist ance are the two most important pathogenic mechanisms of type 2 diab etes.Now there are some studies indicating that a secreted protein named Betatrophin can promote the mice islet beta cells regeneration,impro ve insulin function,and then achieve the goal of improving blood gluco se.Other studies suggest Betatrophin is also involved in lipid metabolis mprocess,which affects the body's blood lipid level.This paper studiedt he correlation of Betatrophin with islet beta cell function,insulin resista nce,lipid metabolism index in individuals with different glucose toleranc e.Methods: This study was conducted in Health Examination Center of Hebei Province General Hospital from January to April,2016.Event ually,a total of 225 individuals were enrolled in the study.After writte n informed consent,oral glucose tolerance test with a 75 g of glucose lo ad was proceeded in all subjects.Peripheral venous blood samples were collected from all participants.Serum was obtained by centrifugation at3,000 rpm for 10 min.Blood glucose,blood lipids and other parameters were analysed by automatic biochemical analyzer.Serum insulin,thyroid hormones and thyroid stimulating hormone were detected by electrochem ical luminescence method.Glycated hemoglobin was detected by highper formance liquid chromatography.Betatrophin was detected by ELISA me thod and the kits were bought from Wuhan EIAab with a model of E11644 h.Liver was detected by ACUSONS2000 ultrasonic apparatus.The subjects were divided into 3 groups:normal glucose tolerance(NGT),i mpaired glucose regulation(IGR),type 2 diabetes mellitus(T2DM).The homeostasis model assessment of insulin resistance(HOMA-IR)was use d to evaluate IR,as calculated by the following formula: Fasting glucos e(mmol/L)×fasting insulin(m U/L)/22.5.The islet beta cell function is ass essed by HOMA-?,HOMA-?=20×fasting insulin(m U/L)/(Fasting glucose(mmol/L)-3.5)(%).All statistical analyses were performed using SPSS 21.0 software.Results:1 The association of the baseline clinical characteristics and differ ent metabolic status.We divided the subjects into 3 groups: NGT 97 people,IGR 72 pe ople,T2 DM 60 people.We observed the differences of gender,age,B MI,WHR,glucose and insulin levels,lipid levels among NGT group an d IGR group,T2 DM group.The results suggested that sex,age and B MI among the three groups had no significant difference(P=0.141,P>0.05,P=0.732).The WHR of T2 DM group was higher than both NGT gro up and IGR group(P<0.01),while there was no significant different bet ween NGT group and IGR group(P>0.05).The lipid metabolism indicators show that compared with NGT gro up,the T2 DM and IGR group have higher TC,TG and LDL-C(P<0.01),while there was no significant difference between T2 DM and IGR g roup(P>0.05).The incidence of fatty liver in T2 DM group is higher tha n NGT and IGR group(P<0.01),while there was no significant different between NGT group and IGR group(P>0.05).The glucose metabolism indicators,indicating FBG,PBG,HBA1 c,FINS and 2h INS among the three groups are significant difference(P<0.01).FBG,PBG and HBA1 c were gradually higher among the NGT gro up and IGR group,T2 DM group(P<0.01).The FINS and 2h INS of IG R group and T2 DM group were higher than NGT group.But only FINS between IGR and NGT group is significantly different(P<0.01).Com pared with IGR group,the 2h INS in NGT and T2 DM were significantly lower(P<0.01).2 Associations between HOMA-IR?HOMA-? and different metabolic status.We observed the differences of HOMA-IR,HOMA-?among the three groups.HOMA-IR was becoming gradually higher among the three g roups(P<0.01).Compared with the NGT group,IGR and T2 DM group have higher HOMA-IR(P<0.01).HOMA-? was becoming gradually lo wer among the three groups(P<0.01).3 Comparison of serum levels of Betatrophin among NGT,IGR an d T2 DM group.Betatrophin became gradually higher among the 3 groups.We foun d there was significant difference among these three groups.Compared with T2 DM group,NGT and IGR group had lower Betatrophin level(P<0.01).The Betatrophin between NGT and IGR group are not significantl y different(P>0.05).4 The correlation of serum Betatrophin with basic parameters,Gluc ose metabolism and lipid metabolism.In the study,we analysed the association of serum Betatrophin with factors related to glucose and lipid metabolism by Spearman's correlatio n coefficient.The results showed that serum Betatrophin was positively correlated with age,WHR,TG,HOMR-IR,FBG,PBG,HBA1 c and FI NS and negatively associated with HOMA-? and HLD-L.However,Bet atrophin had no significant correlation with TC,LDL-C and 2h INS.5 The association between serum Betatrophin level and the prevale nce of fatty liver.We divided all the participants into four groups across Betatrophin quartiles(Q1: 132.57-217.11pg/ml,Q2:217.12-327.11 pg/ml,Q3: 327.12-504.09 pg/ml,Q4: 504.10-1419.62 pg/ml).From the lowest quartile to t he highest quartile of serum Betatrophin level,the prevalence of fatty liver were 41.38%,47.37%,35.19% and 75.0%,respectively.Compared with subjects in the first quartile,a significant increase of prevalence of fatty liver was observed in the highest quartile(P<0.01).Compared wit h serum Betatrophin in quartile 1,univariate logistic regression analysis showed that Betatrophin in quartile 4 had a significant correlation wit h increased odds of fatty liver.After further adjustment for age,sex,B MI,FBG,TG,TC,LDL-C,HBA1 c,the ORs of fatty liver for increasin g serum Betatrophin quartiles were 1.00(reference),4.13(95%CI:1..88-9.05),5.33(95%CI:2.23-12.7)and 21.72(95%CI:2.81-168.11),respectively.6 The association of serum TG level with the prevalence of fatty liver.We divided all the participants into four groups across TG quartiles(Q1: 0.35-1.06mmol/L,Q2: 1.07-1.405mmol/L,Q3: 1.41-2.5nmol/L and Q4: 2.51-mmol/L).From the lowest quartile to the highest quartile of se rum TG level,the prevalence of fatty liver were 33.3%,43.9%,47.3%and 73.7%,respectively.Compared with subjects in the first quartile,a significant increase of prevalence of fatty liver was observed in the hig hest quartile(P<0.01).Compared with serum TG in quartile 1,univariate logistic regression analysis showed that TG in quartile 4 had a signifi cant correlation with increased odds of fatty liver.After further adjustm ent for age,sex,BMI,FBG,TC,LDL-C,HBA1 c,Betatrophin,the ORs of fatty liver for increasing serum Betatrophin quartiles were 1.00(refe rence),13.33(95%CI:5.12-34.74),14.91(95%CI:5.26-42.29)and 54.68(95%CI:7.77-384.58),respectively.Conclusion:1 Serum Betatrophin was positively correlated with insulin resistanc e and triglyceride and negatively correlated with HOMA-?;2 Serum Betatrophin levels were closely associated with developme nt of insulin resistance and fatty liver.
