Background: Acute myocardial infarction(AMI)is one of the critical cardiovascular internal medicine,a serious threat to human health.Left ventricular remodeling(LVR)after acute myocardial infarction is the pathological basis of poor prognosis,such as heart failure after myocardial infarction and sudden death.Early diagnosis and treatment of LVR is the best way to improve the survival quality of patients after AMI.therefore,constantly explore and discover the factors related to LVR after myocardial infarction have important scientific significance.Objective: By measuring the serum levels of CTRP5 and NT-proBNP in patients after AMI,combined with the result of cardiac ultrasound,to research if CTRP5,as a biological marker of LVR after myocardial infarction,can reflect the degree of LVR and predict the development of heart failure.Methods: AMI group included 66 adults from the second affiliated hospital of Hebei medical university between February 2016 to October 2016,they had all been diagnosed with acute ST segment elevated myocardial infarction for the first time.22 adults,excluded coronary heart disease,were selected from the physical examination as the control group in the same period.Detailed records about ordinary situation and past medical history such as age,gender,hypertension,diabetes and hyperlipemia were made in all search objects.Medical examination were well done.The serum levels of Biochemical items,myocardial enzymes,troponin,N-terminal pro-brain natriuretic peptide(NT-proBNP)and CTRP5 were determined.After one month follow-up,all patients in AMI group done a heart ultrasound to record the left ventricular ejection fraction(LVEF),left ventricular end diastolic volume(LVEDV)and E/E'.SPSS 21.0 software was used to statistical analysis,concluding that P<0.05 have statistical significance.Results: CTRP5 level in AMI group(210.55±39.82ng/mL)was higher than that in control group(107.19±11.72ng/mL,P < 0.05).In AMI group,CTRP5 and NT-proBNP levels were positively correlated(r=0.619,P<0.01).CTRP5 level in AMI group was negatively correlated with LVEF(r=-0.473,P<0.01),while positively related with LVEDV(r=0.543,P<0.01)and E/E'(r=0.499,P<0.01).NT-proBNP levels in AMI group was negatively correlated with LVEF(r=-0.753,P<0.01),while positively related with LVEDV(r=0.705,P<0.01)and E/E'(r=0.752,P<0.01).The median level of CTRP5 in AMI group was 206.23ng/mL,on this basis,the AMI group was divided into low CTRP5 group and high CTRP5 group.In low CTRP5 group,the levels of NT-prBNP,LVEDV and E/E' were lower than that in high CTRP5 group(P<0.05),while LVEF level was higher than that in high CTRP5 group(P<0.05).Conclusions: The serum level of CTRP5 in AMI patients is obviously higher than that in healthy people.Moreover,CTRP5,consistented with the NT-proBNP trend,is positively correlated with LVR after myocardial infarction.Prompting that CTRP5,as an important indicator,can be used to predict the degree of left ventricular remodeling after myocardial infarction.
