Value Of Magnetic Resonance Diffussion-weighted Imaging In Diagnosis Of Focal Liver Lesions

Objective To evaluate the value of quantized diffussion-weighted magnetic resonance imaging(DWI)in diagnosis of focal liver lesions.Methods A retrospective analysis was performed in 206 patients with focal liver lesions(FLL)and 35 cases of liver image data who have completed the examination using 3.0T MR,including DWI and routine non-enhanced MRI.Among these 206 patients,there are 298 focal liver lesions,including 59 hepatocellular carcinomas(HCC),67 metastatic tumors,14 cholangiocellular carcinomas(CC),15 hepatic adenomas,11 focal nodular hyperplasias(FNH),44 hemangiomas,26 hepatic echinococcosis and 42 cysts and 20 liver abscess,a statistical difference with p<0.05 was considered significant.Results When the b value is 800s/mm2.(1)57 HCC,60 hepatic metastases,14 CC,15 hepatic adenomas,10 FNH,44 hemangiomas,21 hepatic echinococcosis,18 liver abscess and 5 hepatic cysts showed high signal,the DWI images of malignant lesion group was higher than benign lesion,and significantly different from malignant lesion group(p<0.05);(2)ADC images in HCC,metastatic tumors,CC and liver abscess were cold colour,and in hepatic cysts hemangiomas and hepatic echinococcosis were hot colour,hepatic adenoma and FNH is located between the two,which is similar to that of normal liver parenchyma;(3)9 kinds of liver lesions and normal liver parenchyma have significant difference in ADC value as a whole(p<0.05).Liver cyst(the highest ADC value),liver echinococcosis,liver hemangioma,liver abscess(the lowest ADC value)were compared with the reminder 9 kinds of ADC,there were significant statistical difference(p<0.05),the three types of malignant lesions met in ADC values,there was no significant differences(p>0.05),relating to background liver,comparison relative ADC value of liver cancer with liver metastases were significant difference(p<0.05),however,there was no significant difference in ADC value between hepatic adenoma,FNH and normal liver parenchyma(p>0.05).(4)Relating to the ADC value of 9 tumors and normal liver parenchyma,taking ROC cure to analysis diagnostic efficiency of DWI,the area under ROC,sensitivity,specialty,Youden index in diagnosis differernt metastasas was 0.889?77.85%?96.43%?0.743.Conclusion DWI has a great advantage in the diagnosis of FLL,the DWI images of benign lesion group was lower than malignant lesion,relating to ADC map and ADC value provide important additional information regarding differential diagnosis of specific FLL to some extend.
