Objective To compare the diagnostic performance of 3D-SPACE,a three-dimensional isotropicresolution intermediateweighted fast spin-echo sequence,which can obtain the isotropic data,the meniscus was reconstructed and the diagnosis results were recorded,with a routine magnetic resonance(MR)protocol at1.5 T for meniscal tears of the knee joint.And compare the accuracy of the two in the semi-platform classification.Methods 3D-SPACE was added to a routine MR protocol performed at1.5 T on patients.All cases were routine 2d-sequence,including coronary position,sagittal position,axial 2 d sequence,and 3d space sequence,and underwent subsequent knee arthroscopic to evaluate the tears of internal and external meniscus.Using arthroscopy as the reference standard,compare the diagnostic coincidence rate of meniscal tears between3D-SPACE and the routine MR protocol.And the application value of 3D-SPACE sequence in the classification diagnosis of meniscus tears.Results 1 The specificity of 3D-SPACE sequence value 95.92%,sensitivity value90.57%,positive predictive value 96 %,positive predictive value 96 %,negative predictive value 98 % and accuracy 95 %,respectively.The specificity of 2D-FSE sequence diagnosis of meniscus tears was 92 %,sensitivity was 92.16 %,positive predictive value was 94 %,negative predictive value 96 % and accuracy 93 %.3D-SPACE had similar diagnostic accuracy of the routine sequence(95%/93%,X2=0.355,P>0.05)for detecting mensical tears.2 Three-dimensional sequence obtain the isotropic data,using thinner slice thickness,and can conduct multiplanar reconstruction observation,can better display the details of meniscus tears and the fine structure around the meniscus,for conventional 2d sequence because partial volume effect can not clear ? signal can better display.Chi-square test was used to compare the diagnostic accuracy,X2=0.024,P<0.05,the difference was statistically significant,it can be considered that 3d is more accurate in the grading diagnosis of meniscal injury.Conclusion As a new imaging technique,3D-SPACE sequence obtained high resolution isotropic image,through multiplanar reconstruction,which can more accurately display the anatomical structure of the meniscus and the small structure around,and still can clearly display the meniscus injury less than 1 mm,had significantly advantage atclassification diagnosis of meniscus tears.And so it has certain potential application value in evaluating the meniscua tear of knee joint.
