Clinical Analysis Of LPFP,PFNA And BPH For Treating Femoral Intertrochanteric Fractures In Elderly Patients

?Objective?To evaluation the clinical results of femoral intertrochanteric fractures in elderly patients treatment with locking proximal femur plate(LPFP),proximal femoral nail antirotation(PFNA)and bipolar hemiarthroplasty(BPH).? Methods ? Retrospective analysis of the 86 elderly patients with femoral intertrochanteric fractures during June,2011 and August,2016 were enrolled in this study.The patients were divided into 3 groups: the LPFP group had 10 males,16 females and there are 5 type ? cases,12 type ? cases,and 9 type ? cases which were recording the Evens methods;the PFNA group had 17 males,22 females and there are 11 type ? cases,18 type ? cases,and 10 type ? cases which were recording the Evens methods;the BPH group had 9 males,12 females and there are 1 type ? cases,10 type ? cases,and 10 type ? cases which were recording the Evens methods.The average age of the LPFP group was 72.54±4.78,the PFNA group was 74.41±5.65 and the BPH group was 76.23±6.97.The data of three groups were collected for statistical analysis on the following aspects: operation time,intraoperative blood loss,The length of hospital stay,posteroperative complications and functional outcome,radiographic score and Harris score of the hip joint.?Results?There are 84 cases caught follow-up visits,while the 2 of them was not.The follow up rate is 97.6%.The patients were followed up 22 to 41 months,an average of 26.3 months.(1)In the compare of the operation time and the length of the hospital stay,the BPH group was shorter than the LPFP and PFNA group(F=19.782,21.981;P<0.05),while there was no significant means between the LPFP and PFNA group(T=-0.21,P>0.05).(2)In the compare of the intraoperative blood loss,the BPH group was less blood loss than the LPFP and PFNA group(F=12.976,P<0.05),while there was no significant means between the LPFP and PFNA group(T=0.879,P>0.05).(3)In the compare of the posteroperative complications and functional outcome,the LPFP group was higher than the BHP and PFNA group(F=30.976,P<0.05),while there was no significant means between the LPFP and PFNA group(T=0.902,P>0.05).(4)In the compare of the hip joint after 3 weeks,the BPH group was much higher than the LPFP and PFNA group(F=42.871,P<0.05).And after the 6 months and 2 years,the BPH and PFNA group were both higher than the LPFP group(F=42.871,P<0.05).While there was no significant means between the BPH and PFNA group(T=4.971,P>0.05).?Conclusion?For the elder patient with femoral intertrochanteric fracture,BPH could be the best optional for the shorter operation time and less complications.And the PFNA also could be optional choice for the patients while the LPFP was forbidden for the elder patients.
