ObjectiveThe relationship between periodontitis and type 2 diabetes mellitus(T2DM)is more and more closely, highly attention of the world.Periodontitis may increase susceptibility to systemic diseases such as T2 DM.T2DM is one of the important risk factors of periodontal disease.T2 DM and periodontitis affect each other, not only on the mechanisms, but also on the treatment. The aim of this study is to investigate the treatment effects of clinical periodontal index and blood glucose level after periodontal initial treatment in T2 DM patients with chronic periodontitis. The original treatment plan for blood glucose controlling doesn't change. The aim of this investigation is to provide the theoretical guidance and practical basis for clinical treatment.Methods1.According to the standard of inclusion and exclusion, the present study included 30 patients of type 2 diabetes with chronic periodontitis with age from 45 to 60 years old in the Affiliated Hospital of Qingdao University. All of the patients came from the Department of Periodontology and Endocrinology,Affiliated Hospital of Qingdao University from 2014 to 2016.30 patients were divided into treatment group and control group randomly with 15 cases respectively.2.The control group received only diabetic routine treatment(food, drugs or insulin injections). The treatment group received both diabetic routine treatment and periodontal initial therapy.Periodontal treatment was done by the same student doctor, which is guided by the same professor.French Setelec P5 ultrasound equipment,Gracey curette and UNC-15 periodontal probe were used for the study.3.After 3 months, the fasting glucose levels and periodontal index(such as PD?BI)were detected and analyzed.Statistical analyses were performed using SPSS 17.0.Chi-square test for gender and t test for age were used in this study. The comparison of the glucose levels and the index of PD?BI before and after the treatment were used by the independent sample t-test.Results1.The differences of gender and age are not statistically significant(P>0.05).2.The status of gum is improved after periodontal initial treatment. Before the treatment, the difference of the comparison of the treatment group and the control group for PD?BI are not statistically significant(P>0.05).After the periodontal initial treatment, the difference of the comparison of groups for PD?BI are statistically significant(P<0.05).3.There is no statistically significant of blood glucose levels between the control group and the study group before treatment,P=0.747(P>0.05).After periodontal initial therapy, the glucose level in the study group is significantly decreased compared with the control group,P =0.035(P<0.05).ConclusionIn this study, the clinical periodontal status is improved in the patients with T2 DM with periodontitis by periodontal treatment. It indicates that periodontal initial therapy decrease the periodontal inflammation of patients with T2 DM.Periodontal initial therapy is benefit for controlling of glucose level in study group. For patients with T2 DM,not only the normal blood sugar control measures(such as food, drugs, insulin injections),but also periodontal treatment were needed. To control the level of blood glucose,it is necessary to take actively periodontal initial therapy for patients with T2 DM and periodontitis.Especially in patients with poorly controlled blood glucose levels, they should consider whether they need to take further oral examination and periodontal treatment.
