Objective:Intravenous thrombolysis and mechanical thrombectomy are common and effective treatments. However, there is controversy about the efficacy and safety of mechanical thrombectomy combined with intravenous thrombolysis and mechanical thrombectomy alone in acute ischemic stroke. This meta-analysis was to compare the efficacy and safety of these two methods.Method : Databases(Pubmed,Google schalor,Cochrane library, clinicaltrial,CNKI)were searched for related articles with inclusion exclusion criterion.According to the results of quality evaluation, the fixed effect model was used for meta-analysis.Results:A total of 6 articles were included in the study.692 patients were treated with mechanical thrombectomy combined with intravenous thrombolysis, and 244 patients were treated with mechanical thrombectomy alone. And results are: Mortality(OR=0.70,95%CI 0.49-0.98,P=0.04);Recanalization(OR=0.86,95%CI 0.56-1.33,P=0.51);Hemorrhage(OR=0.94,95%CI 0.48-1.85,P=0.87);m RS(0-2)after 3 months(OR=0.93,95%CI 0.69-1.27,P=0.66).Conclusion:The mortality of mechanical thrombectomy combined with intravenous thrombolysis was low than mechanical thrombectomy alone. While, the recanalization rate and hemorrhage rate and m RS(0-2)after 3 months were no significant difference between mechanical thrombectomy combined with intravenous thrombolysis and mechanical thrombectomy alone.Due to the limited number of patients enrolled in this study,a large randomized controlled prospective study is needed.
