Study Of Left Ventricular Mass Measurement In Patients With Hypertrophic Cardiomyopathy Using Real-time Three-dimensional Echocardiography

Objective:1.To evaluate left ventricular mass(LVM)for patients with hypertrophic cardiomyopathy(HCM)using Real-time three-dimensional echocardiography quantitative technology(RT-3DE),and explore the clinical significance of LVM for HCM patients.2.To detect the overall strain of left ventricular by three-dimensional speckle tracking imaging(3DE-STI),analyse its correlation with LVM and evaluate left ventricular systolic function of HCM cases.Method:1.Object of study: A total of 37 patients diagnosed as HCM by conventional echocardiography were selected as the observation group in our trial(of which satisfactory images were acquired in 33 cases,including 23 males and 10 females,from 32 to 73 years old).And 34 healthy cases were included as the control group(of which satisfactory images were acquired in 27 cases,including 17 males and 10 females,from 24 to 82 years old)2.Equipment: The conventional two-dimensional images were acquired by the United States GE Vivid E7 color Doppler ultrasound diagnostic equipment.Measurements of LVM and left ventricular mass index(LVMI)for HCM patients were accomplished by Penn method of M-mode echocardiography(MME),area length(AL)and trun-cated ellipsoid(TE)of two-dimensional ultrasound(2DE)and RT-3DE,respectively.In addition,3DE-STI was used to obtain the absolute value of strains(LVGLS,LVGCS,LVGAS and LVGRS).The parameters of MME?2DE and 3DE were recorded at the same time.The correlation between LVMs detected by different methods was investigated.And the correlation between LVM and the overall strain results of left ventricular was analysed then.Results:1.Compared with the application of RT-3DE measurement of LVM and LVMI,there was no significant difference of LVMI and LVM measured by MME(P > 0.05),application of 2DE measurement of LVM and LVMI in HCM group showed a statistically significant difference(P < 0.05),showed no statistically significant difference in the control group(P > 0.05).2.Compared with the control group,the result of correlation analysis showed that absolute values of LVGLS,LVGAS and LVGCS all decreased with the increase of LVM in the HCM observation group.This negative correlation was more obvious between LVGLS,LVGAS and LVM,whereas the absolute value of LVGRS was positively correlated with LVM.The same as LVMI.Conclusion:1.MME,2DE and RT-3DE were all feasible for the measurement of LVM.But MME was not effective enough for LVM measurement.3DE could realize real-time measurement of LVM for HCM cases,which was more accurate than2 DE.It also provided strong evidence for the early evaluation and prognostic information of HCM patients.2.LVGLS and LVGAS had significant positive correlation with LVM.With the aggravation of myocardial hypertrophy,longitudinal and areal deformation of myocardium decreased.As an early sensitive indicator for left ventricular systolic function evaluation,LVM measurement provided a new method for clinical diagnosis and management.
