Objective Understanding in patients with acute ischemic stroke patients older than 80-year-old efficacy and safety of intravenous thrombolysis.Method 1.Collection of 274 cases of patients,medication in the onset of 4.5 hours,aged between 60-89,including 184 cases of rt-PA intravenous thrombolysis therapy,and divided into two groups according to age,younger elderly thrombolysis group 96 cases,aged 60-79 years old,give rt-PA intravenous thrombolysis treatment;88 cases year-old thrombolysis group example,the 80-89 years old,or give the rt-PA intravenous thrombolysis treatment;-year-old non thrombolysis group 90 cases,80-89 years old,or give oral antiplatelet aggregation of aspirin enteric-coated metformin hydrochloride treatment,don’t give the rt-PA intravenous thrombolysis.2.Analysis of selected patients’ gender and age,risk factors,hypertension,diabetes,coronary heart disease,hyperlipidemia,coronary heart disease,atrial fibrillation,24 hours at the National Institutes of Health Stroke Scale(NIHSS)score after thrombolytic therapy to improve the situation,24 hours after thrombolysis and thrombolysis rate of hemorrhagic transformation 24 hours after the fatal bleeding rate.The efficiency of treatment,the rate of deterioration and the rate of bleeding conversion were compared.Result 1.Three groups of elderly patients were compared at baseline data,the risk factors,such as hypertension,diabetes,hyperlipidemia,smoking,obesity,gender,blood pressure and blood glucose,baseline NIHSS score had no significant difference;the elderly elderly thrombolysis group and non thrombolysis group had no statistically significant difference in age;the elderly and coronary heart disease and atrial fibrillation were higher than that of the middle aged group,thedifference was statistically significant.2.For the onset of the disease in the 7 days,the short-term effective rate of the thrombolysis group was similar to that of the elderly thrombolysis group,and the difference was not statistically significant.There was no significant difference in the rate of bleeding after thrombolytic therapy in the thrombolysis group and the old age group.Two groups in the onset of 7 days,there was no fatal cerebral hemorrhage.Compared with the old aged thrombolysis group,the former was better than the old one,and the difference was statistically significant.Patients with asymptomatic cerebral hemorrhage in elderly non thrombolytic group.After 7 days,there was no fatal intracerebral hemorrhage in the three groups.3.The effective number of elderly thrombolytic therapy group was 31 cases(35.2%),and the number of effective treatment cases was 2 cases(2.2%)in the elderly without thrombolysis group,the difference between the two groups was statistically significant.There is a significant difference in the effective rate between the elderly thrombolysis group and the elderly non thrombolysis group.4.There is no significant difference between the elderly thrombolysis group and the young elderly thrombolysis group in the occurrence of intracranial hemorrhage transformation,symptomatic intracranial hemorrhage and non symptomatic intracranial hemorrhage.Conclusion Intravenous thrombolysis is safe and effective in patients with acute ischemic stroke over 80 years of age. |
