Sedative Effect Of Propofol Combined With Midazolam In Patients With Mechanical Ventilation: A Meta-analysis

Objective: Respiratory failure is a common disease in patients who were in intensive care unit.Mechanical ventilation is an effective an important treatment in patients with respiratory failure.However,There are many factors can affect the treatment in patients with mechanical ventilation,let patients with anxiety,anxiety,tension,fear,irritability,confrontation and other adverse reactions,lead to exacerbations.Therefore,sedation has become a routine treatment for ICU patients with mechanical ventilation.Propofol and midazolam for sedation is the most commonly used drugs.but when they are used alone,there are some adverse reactions.in order to compare the effect and adverse reaction of midazolam and propofol combined with midazolam.We use the system evaluation and comprehensive analysis on the randomized controlled trials about propofol combined with midazolam and midazolam,in order to provide evidence for its clinical application.Methods: We searched the articles from inception until December 2016 in China national knowledge infrastructure,Chinese Journal of science and technology,Wanfang database,PubMed,Cochrane Database of Systematic Reviews,Embase.search terms are Propofol,midazolam,mechanical ventilation,intensive care unit.The selected articles and other relevant documents were retrieved two times,as far as possible to avoid missing.The data were extracted and the quality of the literature was evaluated,and then the patients were divided into midazolam group and midazolam group according to different intervention measures.Reman5.3 software was used to carry out statistical analysis.Account to the difference of the data,odds ratio,95% confidence interval in each group.Results: A total of 15 randomized controlled trials were included.A total of 13 articles including 908 patients reported effects of midazolamcombined with propofol and midazolam on sedation onset time,and show that the midazolam combined with propofol sedation faster.A total of 13 articles including 1068 patients reported midazolam combined with propofol and midazolam to achieve time that satisfactory sedative,showed that midazolam combined with propofol sedation group reached a shorter time with satisfactory results.A total of 12 articles including 868 patients reported midazolam combined with propofol and midazolam for recovery time after sedation,showed that midazolam combined with propofol sedation group patients after recovery time was shorter.A total of 11 papers,including a total of 813 patients,reported a lower incidence of adverse reactions in the combination of midazolam and propofol.Conclusions: Midazolam combined with propofol sedation to rapid onset,achieve the goal with the shortening of the time,quick recovery after discontinuation of sedative drugs,low incidence of adverse application.improve the safety,effectiveness and economy of midazolam alone application.It is a good sedation strategy in ICU patients.
