Cerebrovascular disease is a kind of disease with high incidence,high morbidity and mortality in china,Often onset of Middle-aged and elderly,To the country and the family caused heavy economic burden and burden of care.The disease includes: hemorrhagic and ischemic cerebrovascular disease.recent years.China's economic costs caused by the disease nearly 20 billion yuan.At the same time,it also brings heavy medical,nursing and economic burden to the family.Has become an important public health issue.At present,the clinical treatment of cerebrovascular disease is not much,It can be divided into drug therapy,rehabilitation therapy,surgical treatment and interventional therapy,With the development of imaging,pharmacy and materials,The rapid development of interventional therapy for ischemic cerebrovascular disease,Technology constantly updated development,More widely used,Endovascular interventional therapy is an effective method for the treatment of ischemic encephalopathy,In this paper,the progress of endovascular intervention in the treatment of ischemic encephalopathy.Objective: The incidence of ischemic cerebrovascular disease increases year by year.The main causes of the disease are intracranial and Extracranial artery stenosis.Traditional treatments include drug therapy,surgery,rehabilitation,etc.But there is still no effective way of avoiding the high lethality and disability caused by ischemic encephalopathy.As involved in the development of technology and popular,vascular intervention especially serious external stent angioplasty is widely used in intracranial artery stenosis caused by ischemic cerebrovascular disease.Because of intravascular interventional therapy is a minimally invasive surgery,this kind of treatment method to brain tissue injury of small,relatively simple operation,high success rate,improve the blood supply of the ischemic brain tissue,and thus received extensive attention of scholars at home and abroad.The Willis' circle often has structural or hemodynamic variations.His incompleteness and hemodynamic abnormalities played an important role in the deelopment of ischemic cerebrovascular disease.Observe the recent,long-term efficacy and complications of vascular intervention.The role of the incompleteness of Willis' circle in the development of ischemic cerebrovascular development.To provide clinical evidence for the treatment of ischemic brain disease.Methods:Selected 175 patients with ischemic cerebrovascular disease in the first hospital of Shijiazhuang from February 2012 to February 2015 were selected as the subjects of this study.There were 88 males and females,aged 43-85 years,with an average age of(+ 3.2)years old.ACI in 97 cases(Cerebral hemisphere infarction in 54 cases,23 cases of cerebellar infarction,Multiple sites of cerebral infarction in 20 cases),TIA 78 cases.All the patients and their families were informed consent.The study was approved and supervised by the first hospital of Shijiazhuang and the ethics committee of Shijiazhuang Central Hospital,The patients were admitted to the hospital for routine intravascular interventional diagnosis and treatment,Endovascular intervention,Implanted stent,Parallel ascending aorta angiography + bilateral cerebral angiography+selective carotid angiography.The results of immediate angiography,complications,follow-up results and vascular variation were recorded.And statistical analysis.Results: A total of 181 stents were successfully implanted in all patients,and the success rate was 100%.Immediately angiography after the intervention and the overall stenosis was significantly improved compared with the preoperative,and the difference was statistically significant(P<0.05).There were 10 cases of patients with decreasing in heart rate and blood pressure postoperative,4 bleeding and hematoma occurred at the puncture site,3 cases patients with transient ischemic attack,the total number of complications occurred in 17 cases,the total incidence rate was 9.7%.All patients were followed up for 12 months.The degree of vascular stenosis was operated after 3 months,6 months and 12 months respectively.The total vascular restenosis rate was 4.9%.During the follow-up period,1 case died,5 cases suffered cerebral infarction,4 cases suffered transient ischemic attack.There were 60 cases of A1 dominant sign in all the subjects,11 cases presented carotid artery blood supply,14 cases presented mixed blood group,and the total vascular variation of Willis loop was 48.6%.Conclusions: Because of the high success rate of interventional treatment,good curative effect,high safety,so to have intracranial vascular stenosis and extracranial stenosis of patients with ischemic encephalopathy is an ideal treatment.The proportion ofWillis' circle incomplete in patients with ischemic cerebrovascular disease was higher.The hemodynamic abnormalities caused by vasculature have an important role in the onset of ischemic cerebrovascular disease.
