Objective: Comparison of cannulated compression screws and dynamic hip screws in the treatment of Pauwels? femoral neck fracture.To provide reference for the treatment of Pauwels?femoral neck fracture by analyzing the clinical curative evaluation indices such as operation time,intraoperative blood loss,Harris hip score and the incidence of postoperative screws refund,internal fixation failure,femoral head necrosis.Effectively reducing postoperative complications and resuming the function of hip joint.Methods:A total of 120 patients with Pauwels? femoral neck fracture were treated by cannulated compression screws or dynamic hip screws in our hospital between January 2010 and September 2015.These cases all had a clear history of trauma,such as fall damages or car accidents.They were randomly divided into two groups,60 cases were treated with cannulated compression screws,age 20-65 years old,average 41.2±13.49 years old.60 cases were treated with the combination of DHS and hollow screws,age 20-64 years old,average 38.45±12.89 years old.All of them voluntarily accepted the internal fixation treatment.The general data of the 2 groups was comparable(P> 0.05).Their data were collected retrospectively to compare the operation indicators including operation time(min),intraoperative blood loss(ml);and the clinical curative evaluation indices such as the incidence of postoperative screws refund,internal fixation failure,femoral head necrosis and Harris hip score.Results:Two groups of patients have been followed up postoperatively,6months to 2 years follow-up time,average 1 year.The operation time and intraoperative blood loss in the cannulated compression screws group(43.88±7.04 min,21.05±4.58ml)were significantly less than those in the DHS group(69.17±5.81 min,152.0±8.73ml)(P<0.05).The Harris hip score for theDHS group(88.58±1.66)was significantly higher than that for the cannulated compression screws group(81.58±2.00)(P<0.05).In cannulated compression screws group,postoperative screws refund were 6 cases(the rate was 10%),it was significantly higher than those in the DHS(P<0.05).and femoral head necrosis were 4 cases,(the rate was 6.67%).Conclusion:For the patients with Pauwels III femoral neck fracture,cannulated compression screws have less operation time,intraoperative blood loss and trauma.But DHS fixation is a better treatment compared with cannulated compression screws,which has less postoperative complications and better hip joint function.
