ObjectiveEpilepsy is one of the common diseases of the nervous system.With the change of biomedical model,the diseases such as cognitive impairment and psychological disorders in epilepsy cause more and more attention.Cognitive impairment,anxiety and depression are the most common,Epilepsy prevalence is affected by a variety of factors,but the specific pathogenesis is not clear yet.Researches have shown that miR-146 a,miR-132 can play a key role in the pathophysiology of epilepsy by regulating inflammatory response or synaptic plasticity,while miR-146 a and miR-132 may play a crucial part in cognitive or psychological disorders.However,there is no report in the expression of miR-146 a and miR-132 in the cognitive impairment,anxiety and depression of epilepsy comorbidity.This thesis was to explore the relationship among miR-146 a,miR-132,related factors of epilepsy and epilepsy comorbidity by observing the expression level of miR-146 a and miR-132 in epilepsy patients,and to provide a theoretical basis for the diagnostic value of miRNA in the comorbidity of epilepsy.MethodsA total of 75 subjects were involved in this study,including 40 patients with epilepsy and35 patients in the control group.MoCA,HAMA and HAMD scales were evaluated in patients with epilepsy and healthy controls.Plasma samples were collected from epilepsy group and control group.The expression levels of plasma miR-146 a and miR-132 were detected by real-time quantitative PCR.Results1.The incidence of cognitive impairment,anxiety and depression in the epilepsy groupwas 62.5%,including the incidence of cognitive impairment was 47.5%,the incidence of anxiety was 40.0% and the incidence of depression was 42.5%.The educational degree showed a positive correlation with comorbidity cognitive impairment.Course of disease,duration and number of medication were negatively correlated with cognitive impairment of epilepsy.Seizure frequency,duration,medication number was positively correlation with epilepsy comorbid anxiety.There was a negative correlation between the educational degree and the depression of epilepsy.Course of disease,and number of medication were positively correlated with depression of epilepsy.2.The expression level of miR-146 a and miR-132 in epilepsy patients was significantly higher than healthy control,and the expression level of miR-132 was significantly higher in epilepsy patients.The expression of miR-146 a and miR-132 was significantly up-regulated in patients with epilepsy at low educational level.The level of plasma miR-132 was significantly higher in patients with epilepsy onset earlier,longer course of disease and duration of onset.There was a negative correlation between miR-146 a,miR-132 and the educational degree,course of disease,and duration of onset were positively correlated with miR-146 a,miR-132.3.The expression levels of miR-146 a and miR-132 in epilepsy patients with cognitive impairment,anxiety and depression were significantly higher than those in patients with epilepsy,at the same time with the increase in the type of epilepsy comorbidity,plasma miR-146 a,miR-132 content also increased.Plasma miR-146 a and miR-132 were negatively correlated with the total score of MoCA,the type of epilepsy comorbidity were positively correlated with miR-146 a and miR-132,and miR-146 a,miR-132 showed a positive correlation with the total score of HAMA,HAMD.Regression analysis showed that elevated plasma miR-132 expression was a risk factor for cognitive impairment,anxiety and depression in epilepsy patients.Conclusions1.Cognitive impairment,anxiety and depression are common in epilepsy patients.Cognitive impairment,anxiety and depression are affected by many factors,including social demographic factors(educational level),biological factors(clinical features of epilepsy),Drug factors(AEDs)and so on.2.MiR-146 a and miR-132 have increased the expression in epilepsy,miR-132 can reflect the severity of epilepsy,miR-146 a and miR-132 have the potential to be biomarkers of epilepsy.3.MiR-146 a and miR-132 can reflect the severity of cognitive impairment,anxiety and depression in epilepsy comorbidities,miR-132 can predict the risk of cognitive impairment,anxiety and depression in epilepsy comorbidities,it can provide a theoretical basis for clinical diagnosis,risk assessment and evaluation of cognitive impairment,anxiety and depression in epilepsy.
