Objective To explore the pathogenic mechanism in gastric cancer and precancerous lesion s,by observing the expression of c-Jun,ATF-2,and the infection of Helicobacter p ylori. Methods The expression of Hp,c-Jun and ATF-2 was comparatively determined b y immunohistochemistry in 177 gastric specimens including 20 cases of chronic superficial gastritis(CSG),20 cases of intestinal metaplasia(IM),28 cases of dysplasia(Dys),36 cases of early gastric carcinoma, and 73 cases of advanced gastric carcinoma.Using semi-quantitative score method to detect the expression of c-Jun,ATF-2,and explore the relationship between each other. Results After immunohistochemistry, each group has the expression of Hp,Hp mainly exist in the gastric muc osa epithelial cells.Hp mainly exists in normal mucosa tissues around cancer, cancer within almost no expression. The Hp demonstrated in the groups of Dys(39.29%)and IM(30.00%)were significantl y higher than those in CSG(5.00%),earl y gastric carcinoma group(2.78%)and advanced gastric carcinoma group(1.39%),respectivel y(P<0.05);each group have the expression of c-Jun,dyeing are located in the nucleus, the c-Jun expression rate in superficial gastritis group, and intestinal group is low,in the early and advanced gastric cancer group is relativel y high, and earl y gastric cancer group is highest. The c-Jun demonstrated in the groups of Dys(21.43%),earl y gastric carcinoma group(52.78%)and advanced gastric carcinoma group(45.21%)were significantl y hi gher than those in CSG(10.00%),respectively(P<0.05);earl y gastric carcinoma group with c-Jun expression rate was higher than advanced gastric carcinoma group(P<0.05);gastric carcinoma group(47.71%)with c-Jun expression rate was higher than no carcino ma group(16.18%)(P<0.05);There was no ATF-2 expression in CSG,while in other four groups are,d yeing are located in the nucleus.low ATF-2 expression rate in Intestinal metaplasia group(10.00%)and Dys group(7.1%),higher ATF-2 expression rate in ear l y gastric carcinoma group(38.90%)and advanced gastric carcinoma group(40.37%);gastric carcinoma group(40.37%)with ATF-2 expression rate was higher than no carcinoma group(5.88%)(P<0.05);(4)in advanced gastric cancer patients,Hp infection positive, the c-Jun and ATF-2 expression are also positive. Conclusion Hp infection have a rel ationship with gastric moucosal epithelium of intestinal metaplasia and atypical hyperplasia; The high-expression of c-Jun and ATF-2 may be involved in the progression and occurrence of gastric carcinoma.
