?Objective?To explore the effect of management mode of stroke unit in primary hospital,and compare with traditional treatment modalities.?SUBJECTS?The patients were randomly divided into two groups:srtoke unit rehabilitation group(SU)and general ward control(GW).152 cases of the SU group were treated in the Stroke Unit of Neurology Department of the people's hospital of Haiyang from January,2014 to October,2015.150 cases of the GW group:came from general wards of Neruology Department of the people's hospital of Haiyang from January,2014 to October,2015.?METHODS?1?InterventionThe SU group were treated according to the criterion to therapeutic referring ?STROKE UNIT?edited by Wang Yongjun and ?China Guiddine forCerebrovascular Disease Prevention and Treatment?edited by Rao Mingli.A multidisciplinary tream consisted of neurological physician,rehabilitation the rapist,physical speech therapist,occupational and primary therapist,psychiatrist nurse check up the patients and established a theraputic schedule within 24 hours.All patients accepted early rehabilitation treatment and health edication.Some receiceived psychological guidance,cognitive trainina or speech learning if desired.The GW group received traditional medical care and nursing.completely or partly conforming to ? China Guiddine for Cerebrovascular Disease Prevention and Treatment?.Some of the patients accepted acupuncture or short-term rehabilitation therapy,but not regularly.2.Evaluation toolA series of questionaires and scales were applied to tassessing patients‘ status in this study,including the past history grade,the concomitant disease grade,the National Institute of Health Stroke Scale(NIHSS),the Barthel Index(BI),general questionnaires self-desiged,including age,gender,marital status,occupation,education level,past history scale and concomitant diseases scale,etc.3.Implement MeasureAll patients were informed consent after admission.The two groups were assessed by past history grade,the concomitant diseases grade,NIHSS,BI,Fugl-Meyer scales by trained investigater.At the end of 2 weeks and 4 weeks after initial treatment,NIHSS,BI,Fugl-Meyer scales were revaluated.All the patients were followed up after discharge.T text,T2 text nonparametric test,repeated-measures ANONA were used in this study.All the satistic analysis was done by SPS11.0 software package or SAS software package.Significant level was ?=0.5.?RESULTS?1.Balance tests: The two groups without siginificant difference in age,occupation,education level,martal status,past disease,clinical type,NIHSS and BI,was comparable.2.Studies on NIHSS:There was no significant statistical difference in NIHSS between two groups at 2 and 4 weeks after onset.But it showed significantly improved in SU group after treatment.3.Studies on BI:There was no significant statistical difference in NIHSS between two groups at 2 and 4 weeks after onset.The performance of ADL in patients treated in SU was significantly better than those in GW.4.Studies on Fugl-Meyer scales:There was no significant statistical difference in NIHSS between two at 2 and 4 weeks after onset.But it showed significantly improved in SU group after treatment.?CONCLUSION?Early rehabiliatation treatment in srroke unit could significantly improve motor function and activites of daily living and improve the quality of life in stroke patients.
