Study On The Advantages Of Single J Tube And Double J Tube In The Application Of Radical Resection Of Bladder Cancer

Objective: To compare the safety and advantage of single J tube and double J tube in the radical resection of bladder tumor,and to provide reference for the choice of ureteral stent.Method: Methods the clinical data of 98 patients withradical resection of bladder tumor from 2013 to 2016 were analyzed retrospectively,All the patients were divided into three groups according to the method of urinary diversion,which were divided into three groups: ureterocutaneostomy group(A group),ileal bladder group(B group)and orthotopic bladder operation(C group).Compared with double J tube and single J tube in three groups of patients the incidence of urinary fistula,urinary tract infection,the incidence of pyelonephritis,ureteral stent shedding and retraction,translocation rate,hospitalization time difference.Result:Therewas no significant difference in age,sex,BMR,preoperative hemoglobin,albumin value,and related complications between the three groups of A,B and C.In group A,there was no urinary leakage in the single J group and the double J tube group,and the urinary tract infection rate was 28.6% and 26.7%,respectively.There were1 cases of pyelonephritis in the single J group,and there were 2 cases of pyelonephritis in double J tube group.The difference was not statistically significant.There were 1 patients with double J tube retraction in the double J tube group at the end of the 2 months after operation.The length of hospital stay was 19 days(range,14-29 days)and 19 days(range,9-39 days).In group B,there were 3 cases of urinary leakage occurred in the single J group,and there were 2 cases of urinary leakage in the double J tube group,the urinary tract infection rate was 36.8% and 40%,respectively.Two groups of patients did not occur pyelonephritis,no stent retraction,displacement.The length of hospital stay was 23days(range,11-42 days)and 23 days(day 15-47)respectively.In group C,there was no leakage of urine in the single J group,4 cases of double J group had urinary leakage,and the urinary tract infection rate was(25%)and(62.5%)respectively.The difference between the two groups was statistically significant(P<0.05).There was no acute pyelonephritis in single J tube group,4 cases of double J group had pyelonephritis(25%),the difference was statistically significant(P<0.05).There was no ureteral stentdisplacement in two groups.The length of hospital stay was 20.5 days(range,16-49days)and 23.5 days(day 16-65)respectively.In the A group and C group,the median hospitalization timeof the single J group was significantly shorter than that of the double J group.The A group was 19 days(range,14-29 days)and 21 days(9-39 days)(P =0.001).The C group was 20.5 days(range,16-49 days)and 23.5days(16-65 days)(P =0.001)respectively.The median hospitalization time was similar between the J group and the J group in the ileal neobladder group,for 23 days(range,11-42 days,15-47 days).Conclusion:Compared with single J and double J tube,there was no obvious advantage in the treatment of ureterocutaneostomy and ileal neobladder.Compared with the double J tube,the single J tube can reduce the rate of urinary leakage and urinary tract infection,shorten the length of hospital stay in the Orthotopic neobladder,which is worthy of choice in clinical practice.
