| Objective To investigate the self-management behavior,hope level and family function of maintenance hemodialysis patients,and to further explore the relationship between self-management behavior and hope level and family function in maintenance hemodialysis patients,and to develop health education for health care workers,Provide psychological support,improve their level of hope and improve the patient’s self-management behavior to provide theoretical basis.Methods A total of 210 patients with end-stage renal disease who were treated in the Blood Purification Center of Qingdao University Hospital from May 20 to August 2016 were selected by convenient sampling method.The patients were investigated by general questionnaire,hemodialysis patient self-management scale,self-efficacy scale,APGAR family function evaluation scale,Herth hope scale,hospital anxiety and depression scale(HAD).A total of 210 questionnaires were distributed and 200 questionnaires were collected.The effective rate was 95.24%.SPSSl7.0 software was used to establish the database and statistical analysis.The statistical methods mainly included descriptive statistics,independent sample t test,variance analysis,Pearson correlation analysis and multiple linear regression analysis.Result1.Self-management behavior of maintenance hemodialysis patients The total self-management behavior of maintenance hemodialysis patients in patients with maintenance hemodialysis was between 37 and 78 points,and the total score of self-management behavior was 58.51±9.85 points,and the average score of self-management of hemodialysis patients was From high to low as a partnership,perform self-care,problem solving,and emotional handling.2.The level of hope for maintenance hemodialysis patients.The mean level of hope for 200 patients with maintenance hemodialysis was(34.96± 4.60)points,the lowest score was 13 points,and the highest score was 45 points.The overall hope level of all patients was at the middle level,with 4 cases(2%)in the lower hope group and 38 cases(19%)in the medium hope group,and 158 cases(79%)in the higher level group.3.Family function in maintenance hemodialysis patients Of the 200 patients with maintenance hemodialysis,4 cases(2%)had severe familial dysfunction;38 cases(19%)had moderate dysfunction;158 cases(79%)had good family function and good overall family function.4.Influencing factors of self-management behavior in maintenance hemodialysis patients Univariate analysis of variance showed that the factors influencing self-management had different effects on different dimensions,such as different educational level,monthly household income per capita,different medical insurance methods and different dialysis years,and the difference was statistically significant(P <0.05).There was no significant difference in the total score of self-management behavior among patients with different age,sex,ethnicity,whether children,marital status,whether they were in the industry,different primary disease,dialysis frequency,and without complications(P> 0.05).5.The relationship between self-management behavior and hope level in maintenance hemodialysis patients The level of hope is positively related to the total score of self-management behavior and each dimension of the self-management(r = 0.422,0.356,0.300,0.325,0.394,P<0.01);the positive attitude of the patients to the reality and the future(T)was positively correlated with the total score of self-management behavior and each dimension of the self-management(r = 0.332,0.347,0.225,0.231,0.286,P <0.01),and the positive attitude(P)was positively correlated with the total score of self-management behavior and each dimension of the self-management(r=0.459、0.369、0.365 、0.320 、0.413,P<0.01).The patient’s intimate relationship with others(I)was positively correlated with the total score of self-management behavior and each dimension of the self-management(r=0.293 、0.197 、0.182 、0.282、0.311,P<0.01).6.The relationship between family function and self-management behavior and hope level in maintenance hemodialysis patients Pearson correlation analysis was performed on self-management behavior and scores of family function in maintenance hemodialysis patients.The results showed that the dimensions of self-management behavior were positively correlated with family function scores(r=0.367,0.278,0.256,0.314,0.293,P <0.01)Pearson correlation analysis was performed on the total score of the desired level of patients with maintenance hemodialysis and the score of each dimension and family function.The results showed that the dimensions of hope were positively correlated with the scores of family function(r= 0.545,0.501,0.542,0.366.P <0.01)7.Multivariate stepwise regression analysis of factors influencing self-management behavior in maintenance hemodialysis patients Multivariate linear regression analysis showed that the educational level,family function and hope level were the influencing factors of self-management behavior in maintenance hemodialysis patients.Conclusion Maintenance of hemodialysis patients self-management behavior,hope level,and family functions are closely related to self-management behavior is not ideal,to be improved.There is a positive correlation between level of hope and self-management,family function and self-management behavior,and level of hope and family function.Therefore,nurses should guide patients to face the disease correctly,to avoid negative psychological,and patients and their families in a timely manner to communicate early detection of family functions in the problems,so that patients receive adequate psychological support and family support,establish confidence in the fight against the disease,Self-management behavior,improve the health of patients,while providing a theoretical basis for health education. |
