ObjectiveTo evaluate the short-term outcomes and the impact of conversion for the old with colorectal cancer.MethodsThe clinical data of 760 cases which were diagnosed tumo ur and the patients were older than 60 years old in the Deparment of Gastrointestinal Surgery of our hospital from January 2013 to December 2015,all of the patients receive radical colorectal surgery,meet the inc luding and excluding criteria and individed into three goups,the group of the laparoscopic surgery,the group of the converted surgery,the group of the open surgery,all of theoperations were performed by senior surgeons.Analyse the reason of the conversion and the short-term outcomes of the three groups,for example the intraoperative blood loss,operating time,the postoperative hospital stay,postoperative complations,such as incis ion complications,pulmonary complications anastomaotic compliactions,reoperation,ileus and other complications.ResultsAmong the inc luding 760 patients studied,there are 518 cases had successfully completed laparoscopic surgery,200 cases had completed open surgery,42 cases patients had the converted surgery with the conversion rate of 7.5%.The reason is mainly for the higher garde of the tumor with tissue adhension and the tumor bulk.General perioperative clinical data have no statistic significance(P>0.05).The surgery time and blood loss of the laparoscopic group is less than the converted group(P < 0.05),the operation time of the open group is less than the converted group but the blood loss have no statistic significance(P>0.05).The rate of the incis ion complication pulmonary complication of the converted group is higher than the laparoscopic group,also the converted group had much more blood loss and operation time than the laparoscopic group(P<0.05).But the rate of the anastomotic complication,ileus,reoperation and other complications is not significant.As for the converted group and the open group,there is no statistic significance for all of the complications(P>0.05).ConclusionLaparoscopic surgery had less complications and safer for the old patients,Laparoscopic surgery have advantages than open surgery,such as less intraoperative blood loss,less postoperative complications,the converted surgery does not confer a worse prognosis compared with the open surgery.
