| Objective:to explore the relationship between R.E.N.A.L nephrometry scoring system and the relevant issues of nephron-sparing surgery.Methods : Clinical data of 130 patients who underwent nephron-sparing surgery(NSS)have been analyzed retrospectively.The correlation between R.E.N.A.L nephrometry scoring and the relevant factors including surgical approach,warm ischemia time,blood loss,operative time of NSS were evaluated.Results:according to the scoring system,there are 62,58,10 cases respectively in low,medium and highly complex group;the number of cases of laparoscopic assisted nephron-sparing surgery is respectively: 59 cases(95.1%),26 cases(44.8%),2 cases(20%);and patients with different grading differ in operation method significantly(P < 0.05).Make stratified analysis according to the scores;laparoscopic-assisted partial nephrectomy’s low,moderate complex tumor thermal ischemia time averaged(18.6 ? 5.8)min and(23.0 ? 5.9)min,the medium of intraopertive blood loss is respectively 20 ml and 50 ml,warm ischemia time and blood loss volume were increased with the increase of grading,statistic difference(P < 0.05).In open nephron-sparing surgery(ONSS),there were significant differences among patients of different scoring in the warm ischemia time,operation time and intraopertive blood loss(P<0.05).There was no significant differences among the scoring system and the perioperative complications(P > 0.05).The postoperative complications were associated with RNS,surgical procedure,warm ischemia time,blood loss and operative time(P<0.05).Conclusions:R.N.E.A.L can objectively and effectively assess the complexity of renal tummor and estimate the risk and complexity of nephron-sparing surgery,which is to improve the treatment of renal tumor. |
