| Objective:Thyroid disease is a common disease of the endocrine system,which is usually manifested as a change in the structure and function of the thyroid gland.Adequate function of the thyroid hormone axis critically depends on the essential trace elements iodine and selenium.Both iodine deficiency and excess can lead to an increase in thyroid disease.Selenium is found in high concentration in the thyroid gland,which is an essential micronutrient in the regulation of thyroid hormone synthesis and metabolism.The changes of selenium levels in the body can affect the benign and malignant thyroid diseases.Studies have shown that selenium supplementation can prevent and treat thyroid disease in selenium deficient areas.It is found that selenium has a correlation with thyroid volume and goiter in iodine deficiency areas;However,the relationship between selenium and thyroid volume is still unclear in iodine sufficient area.The aim of this study was to investigate the nutritional status of selenium and iodine in Shanxi area,and the relationship between thyroid volume and serum selenium,whether selenium nutrition level is a risk factor for thyroid enlargement and thyroid nodules.Methods:We selected residents of Shanxi area using a stratified cluster sampling technique,who are over 18 years old and living in the city for more than 5 years,1,470 subjects were selected for the study.The survey first filled in the informed consent form,then fillde the questionnaire,and physical examination,serum thyroid function test,thyroid ultrasound,urinary iodine,and the serum selenium test.Except pregnant women and lactating women,who has a history of thyroid gland and family history,and received iodine contrast examination or taking amiodarone and selenium in the past three months,ultimately,1,339 subjects were included in our study.We analyzed the data using spss22.0 statistical software.The measurement data are expressed as mean ± standard,median,interquantile range,and percentiles.Linear regression and multiple linear regression were used to study the relationship between thyroid volume and blood selenium concentration and its influencing factors.Logistic regression analysis was applied to study whether blood selenium are risk factors of nodules and goiter,when P < 0.05,the difference was statistically significant.Results:1.The median urinary iodine was 216.4 ug/l((interquantile range [IQR]:(139.9,303.6)in the survey population,indicating that the iodine status was more than adequate.2.The median serum selenium level was 78.21(IQR : 58.46,128.77)μg/l,indicating that Shanxi area belongs to selenium deficient.3.The median thyroid volume was 7.52(IQR: 5.90,9.39)m L;In the Spearman Correlation,there were inverse correlation between serum selenium and age(r=-0.088,P=0.001).We found no association between serum selenium and body mass index,and thyroid-stimulating hormone(r=-0.007、P=0.801;r=-0.051、P=0.063).Different selenium levels have no influence on the standardization volume,and no difference of the incidence of the goiter.4.In the linear regression analyses,we found no association between thyroid volume and serum selenium(β=0.007,P=0.367).5.There were no significant differences in serum selenium between persons with or without goiter or nodules(P=0.278,OR=1.173;P=0.399,OR=1.073).Conclusion:1.The level of serum selenium in Shanxi area was in the condition of marginal nutrition.2.This study suggests that selenium has no significant correlation with goiter and thyroid nodules. |
