Objective:To explore the relationship between the level of plasma ADAMTS4 and the vulnerability of carotid atherosclerotic plaque in patients with carotid artery stenosis and the clinical value of evaluating the vulnerability of the carotid atherosclerotic plaque and the risk of stroke incidence.Methods:Sixty inpatients with carotid stenosis and needed to receive surgery therapy from The Second Hospital Of Shan'xi Medical University were enrolled in this prospective study.The patients were selected into stable plaque group and vulnerable plaque group according to the histopathological typing after carotid endarterectomy(CEA).30 patients from the physical examination center who had atherosclerosis but no carotid plaque formation were selected into the control group.Using ELISA to measure the level of plasma ADAMTS4.Statistics analysis were conducted by ANOVA?Pearson chi-square test?Logistic correlation?ROC(receiver operating characteristic curve)as needed.Results:The level of plasma ADAMTS4 was significantly higher in the vulnerable-plaque group(p<0.001)compared with the stable plaque group and the control group,The level of plasma ADAMTS4 in the stable plaque group was higher than the control group,but there is no significant difference(p>0.05).Logisticregression curve showed that the rate of vulnerable plaque in high-plasma ADAMTS4 group was significantly higher than thatin low-plasma ADAMTS4 group(p<0.05,OR=8.240,95%CI=1.512-22.915);the rate of vulnerable plaque in symptomatic group was significantly higher than that in asymptomatic group(p<0.05,OR=12.429,95%CI=3.601-42.902.ROC curve analysis showed that the AUC of plasma ADAMTS4 was 0.943,when cut-off value was set at100.935ng/ml,the sensitivity and specificity were 83.3% and 92.4% respectively.Conclusion:ADAMTS4 is an effective,sensitive and non-invasive biomarker to evaluate the vulnerability of carotid atherosclerotic plaque and the risk of stroke.
