Hemodynamic Responses In Patients With Hypertension Associated With Awake Endotracheal Intubation Using Intubating Laryngeal Mask

Objective:To evaluate the hemodynamic effects in patients with Hypertension associated with awak endotracheal intubation using intubating laryngeal mask.Methods: Sixty Patients,male 32 and female 28,with American Society of Anesthesiologists(ASA)physical status II or III,aged 42-73 yrs,the BMI scores ranged from 21.3 to 27.8,underwent elective hypogastrium surgery under general anesthesia were enrolled and randomly allocated equally into two groups(n=30 each): Group DLS and Group ILMA.After the routine sedation,awake endotracheal intubation was performed with the direct laryngendoscope or intubating laryngeal mask.Respective mean arterial blood presurre(MAP),heart rate(HR)and blood concentration of adrenaline(AD)were recorded at baseline(T1),3min after superfacial anesthesia(T2),insertion of DLS or ILMA(T3),endotracheal intubaiton(T4)and the highest MAP of HR afeter intubation(T5).The one-time success of intubation and the adverse effects in two groups were recorded.Results: There were no significant differences at the aspect of general status of the two groups.Inter-group comparison: the MAP,HR,AD at T3-5 time points in group DLS were higher than that in group ILMA(P <0.01).Intra-group comparison: MAP,HR,AD values at T3-5 time points were higher than that of T2 in group DLS(P <0.01).No significant differences about these values of comparison were obversed in group ILMA.Seven patients complained pharyngalgia or hoarseness in group DLS and only one patient complained these adverse effects in group ILMA(P <0.05).Conclusions: Compared with direct laryngendoscope,hypertensive patients undergoing awake intubating endotracheal intubationg with intubting laryngeal mask render more stability hemodynamics,less stress reaction,less adverse reaction and better clinical results.
