ObjectiveIt aims to discuss the infection distribution of helicobacter pylori in gastric cancer group at Xianyou county,to discover the possible environmental risk factors,to research the associated between related nodes gene polymorphismn of NF-?B signaling pathway and gastric cancer,cardia cancer,noncardia gastric cancer,to explore the possible interaction effects between gene-gene and gene-environmental factor.Methods1.The current situation of Hp infection of gastric cancer and its risk factors.Collectiing 622 newly diagnosed cases of gastric cancer which was diagnosed by histopathology in xianyou hospital in fujian province,selecting 480 patients which had complete data of helicobacter pylori,stratified by the gastric cancer adjusting mortality rate and divided into high,medium and low township.Rapid urease test was used to detect the helicobacter pylori infection in gastric cancer group,obtaining the information such as the risk factors of helicobacter pylori infection through the questionnaire survey,adopting the chi-square analysis to verify the the distribution of helicobacter pylori risk factor between different parts of the gastric cancer,making the multiple-factor analysisusing by unconditioned logistic regression analysis method.2.Applicating the bioinformatics technology and high throughput data to screen gene polymorphic sites in NF-?B noncanonical signaling pathway.1:1 matched case-control study method,according to gender,age,villages and towns matching the healthy people as control with 550 gastric cancer cases,using the method of molecular epidemiology,acquisiting 5 ml blood sample of cases and control group,extracting DNA,applicating Sequenom technology platform to test the genotypes,using conditional Logistic regression analysis to count the relationship between each gene genotypes and gastric cancer,cardia cancer,the noncardia gastric cancer susceptibility.3.Through the conditional logistic regression analysis to obtain the environmental risk factors which had statistical significance of 622 gastric cancer cases and 622 healthy controls,synthetically applicatiing the generalized multifactor dimensionality reduction method,logistic regression analysis and fork analysis principle to discover interaction effect of gene-gene and gene-environment in different parts of the gastric cancer.Result1.Helicobacter pylori infection situation and the distribution features of the risk factors in gastric cancer population:480 cases of gastric cancer patients was in survey,238 patients were HP positive,HP positive rate was 49.6%,on the medium level in fujian area,male infection rate was 48.4%,female positive rate was 52.7%,the infection rate of 45~55 gastric cancer population(55.6%)was the highest,the helicobacter pylori risk factors are: drink the non-tap water,the earliest pesticide exposure age < 20 years old,heavy smoking,eating spicy food,beef and mutton,pickles,family history of cancer,chronic atrophic gastritis,dissatisfied with family status;protective factors are:eating eggs,fresh fish and shrimp,shell snails,kelp laver,bean products,fresh fruits,green Chinese onion and onion.2.The association between the gene polymorphisms of NF-?B noncanonical signaling pathway and gastric cancer: CD40 rs3765459 gene polymorphism had correlation with gastric cancer,AA genotype was a poor genotypes,compared with the people who carried the GG genotype,gastric cancer risk is 1.62 times,this difference was also seen in the noncardia gastric cancer;TNFRSF11A rs1805034 gene polymorphism(TC + CC)dominant model could reduce the cardia gastric cancer risk(OR=0.661,95%CI:0.661,0.931).TRAF3 Grs8022770Crs12878111 gene haplotype could reduce the risk of gastric cancer,cardia cancer,noncardia gastric cancer,the OR value was: 0.105,0.207,0.043.3.The combined effects and interaction effect analysis of gene-gene and gene-environment : according to the result of the joint action of rs7290134 with hard food,the risk of gastric cancer of the person who carried GA or GG genotype and ate hard food your was 4.577 time than the person who carried AA genotype and ate soft food,95% CI: 2.776 ~ 7.545;at the same time,the risk of gastric cancer of the person who carried AA genotype and ate hard food was 4.490 times than that ate soft food,95 % CI: 3.166 ~ 6.369.In the combined effects of rs1805034 wieh soft or hard food,the risk of gastric cancer of the person who carried wild homozygous genotype TT and ate hard food could increase the risk of noncardia gastric cancer than that carried TC or CC genotype and ate soft food(OR95%CI: 6.099(3.406,10.919)).Carrying the same TT genotype,the risk of gastric cancer of the person who ate hard food was 4.490 times than that ate soft food(95%CI:2.440,2.440);In the combined effects of rs1805034 with smoking,carrying rs1805034 TT genotype and smoking in pack-years ? 20 could increase the risk of noncardia gastric cancer(OR =1.810,95%CI: 1.007~3.252).Rs7290134 with hard or soft food,rs1805034 with hard or soft food and smoking pack-years didn't exist multiplication,additive interactions between them.Conclusion1.Helicobacter pylori positive rate of gastric cancer population in xianyou area was 49.6%,the main risk factors were:1.the earliest pesticide exposure age < 20 years old,heavy smoking,eating spicy food,beef and mutton,pickles,drink the non-tap water,family history of cancer,chronic atrophic gastritis,dissatisfied with family status;protective factors are:eating eggs,fresh fish and shrimp,shell snails,kelp laver,bean products,fresh fruits,green Chinese onion and onion.2.CD40 rs3765459 and TNFRSF11 A rs1805034 in the NF-? B noncanonical signaling pathway were associated with gastric cancer susceptibility.TRAF3Grs8022770Crs12878111 haplotype gene could reduce the risk of gastric cancer.The correlation of rs3765459 and Grs8022770Crs12878111 haplotype with gastric cancer had not been reported ever.3.There were interaction effect between gene-environment,the person who carried TNFRSF13 C rs7290134 GA or GG genotype and hard food can increase the risk of gastric cancer,which had not been reported ever.TNFRSF11 A rs1805034TT genotype associated hard food or smoking package years?20 can increase the risk of the risk of noncardia gastric cancer,which suggested that we can control the the disease risk of high-risk population by strengthen the intervention measures for environmental risk factors,thereby forming a comprehensive specific network to prevent and control the gastric cancer.
