Objective: To investigate the diagnostic and prognostic significance of HMGB1?TREM-1?s CD163?CRP?IL-6?TNF-? in patients with hepatocellular carcinomaMethods: A retrospective study was conducted to collect 86 patients with primary hepatocellular carcinoma(HCC)who were initially hospitalized during April 2008 to April 2016 in the First Affiliated Hospital of Fujian Medical University,Department of Hepatology and Hepatology.Forty patients with cirrhosis were selected as cirrhosis group and 40 patients with chronic hepatitis B as chronic hepatitis B group.Serum levels of HMGB1,TREM,s CD163,CRP,IL-6 and TNF-? were collected from the serum of the patients.The clinical data were recorded.Statistical analysis was performed using SPSS19.0 software.The differences of inflammatory factors between the two groups were analyzed by t test,chi-square test and one-way ANOVA.Independent risk factors were selected by Logistic regression model to establish a new prediction model.Evaluate the prediction model using the ROC curve.Independent risk factors were screened by COX multiple regression.P <0.05 indicates that the difference was statistically significant.Results: 1,s CD163,IL-6,TNF-?,TBIL,ALT,AST,GGT,ALP,ALB,PT and AFP were the risk factors of hepatocellular carcinoma.We found that high TREM-1,AFP and low PT were independent risk factors for HCC(P = 0.034,0.002,0.017,respectively).(AUROC = 0.763,P <0.001)was higher than that of AFP(AUROC = 0.762,P <0.001).The diagnostic value of TREM-1 was significantly higher than that of AFP(AUROC = 0.763,P <0.001)The 3,ROC curve area analysis TREM-1 in early liver cancer and non-liver cancer AUROC 0.763,P <0.001,and TREM-1 in early liver cancer and liver cirrhosis AUROC 0.796,P <0.001.4,high concentrations of CRP,IL-6 and TNF-? have poor prognosis of liver cancer.5,multiple COX regression found that TREM-1,s CD163 for the prognosis of liver cancer independent risk factors(P values were 0.025,0.002).Conclusion: Conclusion: 1.High TREM-1,AFP and low PT are independent risk factors for hepatocellular carcinoma.2,TREM-1 combined with AFP can improve the diagnostic value of HCC.3,TREM-1,s CD163 as the independent prognostic factors of liver cancer.
