Safety Of Octogenarian Patients Operated Within 1 Week After Coronary Computed Tomographic Angiography(CTA)

Objective:The aim of the study was to investigate safety of the different time interval from coronary CTA to non-heart vascular surgery in octogenarian patients.Methods: Octogenarian in-patients,who had been operated not heart vascular surgery after being performed coronary CTA from September 31,2015 to October 1,2016 in Fujan Medical university Union hospital,were retrospectively analyzed.According to the different interval time from coronary CTA to operation,the octogenarian in-patients were divided into group 1(operated within 1 week after coronary CTA,n=73)and group 2(operated 1-3week after coronary CTA,n=35).The baseline clinical characteristics,perioperative medication,serum creatinine(Scr)of before coronary CTA and postoperation,urea,acute kidney injury(AKI)and estimated glomerular filtration rate(eGFR)were compared in two groups.The Revised Cardiac risk index(RCRI)was estimated by total number of the preoperative risk factors for each octogenarian in-patient,and compared in different types of non-heart vascular surgery to assess specifically safety.Results: 1.A total of 108 cases(83.1±2.9years,75 men)were enrolled in the study.The group1(n=73,83.1±3.0years,48 men)and the group2(n=35,83.2±2.6years,27 men).2.All of the patients performed coronary CTA,only 1 patients developed palpitation and 3 patients had injection site pain,and other adverse reactions were not found in the study.RCRI ? 2 was found in all types of non-heart vascular surgical patients,which were not death after being performed coronary CTA and operation in the study.thus3.There was not statistical difference in baseline clinical characteristics,Scr of before coronary CTA and postoperation,urea,and eGFR between Group 1 and Group 2(p>0.05).AKI was not found in any one case in the study.4.The changer of renal function before and after surgery in octogenarian patients with different comorbidities(hypertension,dyslipidemia,coronary heart disease).After operation,octogenarian patients with hypertension and dyslipidemia,the level of Scr and eGFR was lower and higher than before performed coronary CTA respectively(p=0.008,p=0.044;p=0.016,p=0.030).However,there was no significant difference in the level of urea before performed coronary CTA and after operation(p>0.05).There was no significant difference in the level of Scr,Urea and eGFR before performed CTA and after operation in octogenarian patients with coronary heart disease(p>0.05).Conclusion: Operation within 1 week after coronary CTA are safety in octogenarian patients with RCRI ? 2 with non-heart vascular surgery.
