?Objective?To Analyse the differences between clinical findings and magnetic resonance imaging(MRI)in diagnosis of abducent nerve involvement in nasopharyngeal carcinoma(NPC),To explore the abducent nerve invasion in NPC Magnetic resonance imaging(MRI)signs.?Methods?666 newly diagnosed NPC patients were scanned with MRI in Fujian Provincial Tumor Hospital.To study naive NPC patients underwent MRI who were diagnosed cranial perineural invasion by MRI.MRI images were analyzed by two radiologists.We explore the incidence rate of MRI diagnosis and clinical diagnosis respectively.We analysis the nerve invasion situation of image and clinical,Chi-square test was used for statistical analysis.We also use Chi-square test to compare the constituent ratios among the different segments of the abducent nerve??Results?1.In 666 cases of NPC,180 cases were cranial perineural invasion by MRI diagnosis,the occurrence rate was 27.03 percent;92 cases were diagnosed Abducens nerve invasion.61 cases appeared cranial nerve palsies,the occurrence rate was 9.16 percent.16 cases were diagnosed Abducens nerve invasion.1 cases of whom appeared Abducens nerve palsies without Abducens nerve invasion of the MRI features,There is significant difference of the incidence rate between the image and clinical diagnosis of nasopharyngeal carcinoma with cranial nerve invasion(P<0.05).2.In the patients with abducens nerve invasion,there is significant difference of the coincidence rate among the cavernous sinus segment ?petroclival segment?cisternal segment and Orbital segment.with 98.92%(92/93)?93.55%(87/93)?3.26%(3/93)?15.05%(14/93)respectively.No significant difference was observed in the cavernous sinus segment ?petroclival segment.The cavernous sinus segment is remarkably higher than cisternal segment and Orbital segment.with significant difference(P < 0.05).The petroclival segment is remarkably higher than cisternal segment and Orbital segment.with significant difference(P<0.05).?Conclusions?1.The incidence of MRI-detected nasopharyngeal carcinoma with cranial nerve invasion(27.03%)is higher than cranial nerve palsy(9.16%).2.The results in the abducens and trigeminal nerves being the most frequently affected.Almost all patients with abduces nerve palsies could have MRI evidence of abduces nerve involvement,especially in cavernous sinus and petroclival segments.3.The way in abducens nerve invasion of NPC,radiologists should be vigilant for clivus and petrous apex abnormalities in setting of abducens nerve palsy.
