The Effect Of Specialized Colorectal Surgery On Surgical Quality And Prognosis Of Rectal Cancer

Objective To investigate the surgical quality and prognosis of rectal cancer after separating the Colorectal Surgery from General Surgery.Methods Making retrospective study on 2696 rectal cancer patients,which are earlier group 1364 patients from oncological surgery before separating the Colorectal Surgery from General Surgery(2000 to September 2010)and later group 1332 patients from colorectal surgery after separating the Colorectal Surgery from General Surgery(from 2010 October to September 2015)included.Making comparision and analysis on lymph node dissection,surgery situation,postoperative recovery,rate of complication as well as the cumulative survival rate of 5 years on those two group.Results Compare to the earlier group,the later one clean much more quantity in lymph gland during T1-3(19.5±9.5 vs.18.7±7.2),it is no significantly differences.much more positive lymph gland(1.7±4.1vs.1.6±3.5),higher anal keeping rate(92.5% vs.69.7%),total complications(15.0% vs.19.2%),anastomotic bleeding(1.4% vs.3.5%),anastomotic leakage(1.8%vs.3.5%),intestinal obstruction after surgery(2.6% vs.5.7%),blood loss during surgery(73.4(70)ml vs.117.2(110)ml),lower operative time(199.9(68)minutes vs.216.6(74)minutes),lower postoperative hospital stay(9.9(2)days vs.13.4(6)days),higher postoperative rectal resection syndrome(ARS)(73.2% vs.87.2%),lower local recurrence rate(2.0% vs.4.3%%),metastasis rate(7.2% vs.17.6%),higher 5 years survival rate after surgery(83.4% vs.73.8%),between earlier group and later group were significantly different(all P<0.05),while there were no significantly differences in celiac leakage,urinary retention,incision infection,pulmonary infection(all P>0.05).Conclusion Separating the Colorectal Surgery from General Surgery could higher the operative quality,lower the complications after surgery,recovery fast after the operation,shorten the hospital stay time as well as higher the long-term prognosis survival rate.However,postoperative rectal resection syndrome correspondingly increase with the high anal keeping rate.
