Objective To investigate the clinical value of using real-time shear wave elastography(SWE)to evaluating the hidden progress of liver fibrosis in Chronic hepatitis b patients with noninvasiveness and quantification.Methods 827 cases of CHB patients were diagnosed clinically.Routine hematology,S WE and liver biopsy were examined by biopsy.Hematological examination ALT,AST low er than the lower limit of detection and imaging examination for the experimental group,A LT,AST and / or imaging abnormalities as the control group.The correlation between the degree of liver elastic modulus and the degree of liver fibrosis in CHB patients was analyz ed by using the pathological criteria as gold standard.Results Of the 827 CHB patients,the hematologic examination was lower than the lo wer limit of detection,644 cases were normal in the imaging examination,and 183 cases were abnormal ALT,AST and / or imaging.There was no significant difference in sex rat io between CHB patients with different stages of liver fibrosis,but the differences in age a nd liver elastic modulus were statistically significant.There was a positive correlation betwe en the elastic modulus of liver and the hematologic examination,and the Spearman correlat ion coefficient was 0.665 and P = 0.000.SWE technique diagnosis hematology examination lower than the lower limit of detection,normal imaging of CHB patients with progressive degree of liver fibrosis S ? 3 threshold of 7.96 Kpa,specificity 80.2%,sensitivity of 83.5%,the area under the curve of 0.889;ALT,AST and / or imaging abnormalities CHB pa tients with progressive liver fibrosis degree of S ? 3 threshold of 8.59 Kpa,specificity of 70%,sensitivity of 75.9%,the area under the curve of 0.856.The correlation coefficient be tween S0 ~ S4 staging group was 0.901,0.917,0.945,0.979,0.891,and the correlation betwee n the two groups was 0.901,0.917,0.945,0.979,0.891.Conclusion 1.SWE technique to diagnose ALT and AST below the detection limit,imaging examination of normal CHB patients with liver fibrosis ? S3 curve area under 0.889,indicating that SWE technology on ALT and AST lower than the detection limit,imaging normal CHB patients with High diagnostic performance,and can provide accurate and objective basis for clinical,non-quantitative assessment of CHB patients with progressive degree of progressive liver fibrosis.2.Liver function transaminase markers AST and ALT can affect the measurement of liver elastic modulus by SWE technique.
