OBJECTIVE: To investigate the effect of changes in mediastinal window and lung window parameters on the measurement of solitary pulmonary nodules.Data: collected from May 2013 to June 2015 in Tianjin Binhai New Area Dagang Hospital 64-slice spiral CT examination of non-definitive pulmonary nodules in 50 cases.(2)long and short diameter are less than 20 mm,and not with the surrounding pleura and blood vessels connected;(3)no calcification and empty Solid nodules;(4)no history of malignant tumors.50 cases of nodules in 10 cases were located in the pleural and pleural closely related,there are 6 cases of nodules with small spots like calcification,4 cases associated with peripheral blood vessels,and ultimately a total of 30 cases meet the criteria so included in the study,including 17 cases of male,Female 13 cases,the age range of 37 years old-78 years,the average age of 58.6 ± 8.6 years.METHODS :All cases had no history of malignancy.Transfer the reconstructed data to the workstation for measurement.(350HU,40 HU and 500 HU,40HU),referred to as mediastinal window 1 and mediastinal window 2;fixed lung window level,change window width(700HU,-550HU;1000HU,-550HU;1500HU,-550HU;1700HU,-550HU),referred to as lung window 1,lung window 2,lung window 3 and lung window 4,first find the largest section of the nodal image,respectively 6 for each nodule in the measurement of the nodule of the long and short diameter,each nodule were measured three times,and the average of the three measurements as the final study data.RESULES: The diameter and short diameter of pulmonary nodules were measured under the parameters of 6 window widths under soft tissue algorithm and lung algorithm.(1)soft tissue algorithm under the conditions: median longitudinal 1 and mediastinal window longitudinal 2 using t test(long diameter t=0.166,short diameter t=0.017,p> 0.05);lung window longitudinal 1,lung window longitudinal 2,lung window longitudinal 3,lung window longitudinal 4 between the use of F test(long diameter F=1.90,short diameter F=3.42,p> 0.05);In the mediastinal window and lung window,the F test(long diameter F = 6.42,short diameter = 7.31,p> 0.05);and then mediastinal window 1 and mediastinal window longitudinal 2,respectively,with the lung window longitudinal 1,lung window longitudinal 2,lung window longitudinal 3,lung window longitudinal 2 two t test: mediastinal window 1 with lung window 1(longitudinal diameter t=2.708,short diameter t = 2.239,p <0.05),mediastinal window longitudinal 1 and lung window longitudinal 2(long diameter t=2.967,short diameter t=2.154,p <0.05),mediastinal window longitudinal 1 and lung window longitudinal 4:((long diameter t=3.122,short Diameter t=2.222,p <0.05);mediastinal window longitudinal 2 with lung window longitudinal 1(longitudinal diameter t =2.425,short diameter t=2.20,p <0.05),mediastinal window longitudinal 2 and lung window longitudinal length 2(long diameter t = 2.82,short diameter t = 2.118,p <0.05),mediastinal window longitudinal 2 and lung window longitudinal 3(long diameter t=2.708,short diameter t=2.176,p <0.05);mediastinal window longitudinal 2 and lung window longitudinal 4(long diameter t = 2.98,short diameter t = 2.205,p <0.05).There was no statistically significant difference between the mediastinal window and the mediastinal window,and the measurement results between the medial wall and the lung window were statistically significant.(2)under the conditions of the lung window algorithm: mediastinal window lung 1 and mediastinal window lung 2 using t test(long diameter t=0.144,short diameter t = 0.026,p> 0.05);lung window lung 1,lung window lung 2,lung window lung 3,lung window lung 4 between the use of F test(long diameter F=2.14,short diameter F=3.57,p> 0.05);The longitudinal test was performed by F test(long diameter F=6.73,short diameter F= 6.93,p> 0.05)between mediastinal window and lung window.And then mediastinal window lung 1 and mediastinal window lung 2,respectively,with lung window lung 1,lung window lung 2,lung window lung 3,lung window lung 2 two t test: mediastinal window lung 1 and lung window lung 1(long diameter t=2.467,short diameter t=2.152,p <0.05),mediastinal window lung 1 and lung window lung 2:(long diameter t=2.826,short diameter t=2.059,p < 0.05),mediastinal window lung 1 and lung window lung 3(long diameter t=2.642,short diameter t=2.212,p <0.05);mediastinal window lung 1 and lung window lung 1(long diameter t=2.975,short diameter t = 2.173,p <0.05);mediastinal window lung 2 and lung window lung1(long diameter t=2.325,short diameter t=2.114,p <0.05),mediastinal window lung 2 and lung window lung 2(long diameter t=2.685,short diameter t=2.029,p <0.05);medial window lung 2 and lung window lung 3(long diameter t=2.501,short diameter t=2.117,p <0.05),mediastinal window lung 2 and lung window lung 4(long diameter t=2.811,short diameter t=2.136,p<0.05).There was no statistically significant difference between the mediastinal window and the mediastinal window,and the measurement results between the medial wall and the lung window were statistically significant.(3)Comparison between soft tissue algorithm and lung algorithm: Mediastinal window 1 and mediastinal window lung 1:(long diameter t=0.166,short diameter t=0.017,p> 0.05);comparison between mediastinal window longitudinal 2 and mediastinal window lung 2:(long diameter t=0.089,short diameter t=0.058,p>0.05);lung window longitudinal 2(t=0.08,p> 0.05);lung window longitudinal 1 and lung window lung 1:(long diameter t=0.154,short diameter t=0.032,p> 0.05);comparison between lung window longitudinal 3 and lung window lung 3:(long diameter t=0.051,short diameter t=0.042,p> 0.05),between the mediastinal window and the mediastinal window,and between the longitudinal window and the medial wall,There was no statistically significant difference between the lung window and the lung window.Conclusion: Under the condition of soft tissue algorithm and pulmonary algorithm,the window position of the longitudinal and window windows was fixed,and the window width was changed within the allowable range.On the reconstructed layer thickness of 1mm,the solitary pulmonary tuberculosis with long and short diameter less than 20 mm Section measuring its long and short diameter.There was no significant difference between the mediastinal window and the mediastinal window and between the lung window and the lung window,and there was a significant difference between the lung window and the mediastinal window in comparison with the results obtained by the soft tissue algorithm.Compared with the results obtained by the lung algorithm,there was no significant difference between the mediastinal window and the mediastinal window and between the lung window and the lung window,and there was a significant difference between the lung window and the mediastinal window.Compared with the results obtained by the soft tissue algorithm and the lung algorithm,there was no significant difference in the results of the same mediastinal window and lung window parameters.Therefore,in the CT examination of solitary pulmonary nodules due to follow the following principles:(1)pulmonary nodules of the CT measurement should choose the same window width,window;(2)pulmonary nodules follow-up observation process,The window is fixed to avoid measurement deviations.
