Objective:Through the detection of Helicobacter pylori infection and Creactive protein,to study its correlation with abdominal type of HenochSchonlein Purpura in children,to provide a theoretical basis for the clinical treatment of abdominal type of Henoch-Schonlein Purpura in children,in order to advance the curative effect, alleviate the clinical symptoms, improve the prognosis and enhance the quality of life. Methods: 1 Cases and groups This research was collected Eighty cases hospitalized in the pediatric kidney and immune rheumatism specialist group in our hospital from september 2015 to December 2016 and diagnosed the Henoch-Schonlein Purpura in children.Forty cases of those were the abdominal type of Henoch-Schonlein Purpura(abdominal type of group),the remaining 40 cases were non abdominal type of Henoch-Schonlein Purpura(non abdominal type of group),and selected healthy children to hospital physical examination as a control group of 32 cases.Children with abdominal type of Henoch-Schonlein Purpura with positive Helicobacter pylori and elevated serum C reactive protein were randomly divided into routine group and treatment group.2 Sample collection, handling and the date measurement The selected children were in the morning 8:00 fasting 13 C urea breath test and children with Henoch Schonlein purpura fasting venous blood about 2ml in the coagulation tube to detect C reactive protein. The routine group was treated with anti allergic and anticoagulant therapy, while the treatment group was treated with anti Helicobacter pylori therapy on the basis of routine treatment. The routine group and treatment group according to the treatment above the corresponding treatment after 5 days, the morning fasting venous blood 2ml in coagulation tube in recheck C reactive protein.Routine group and treatment group in accordance with the corresponding treatment for 14 days, after the withdrawal of treatment for the first time in the outpatient department of Pediatrics of the hospital for 1 months, the morning fasting 13 C urea breath test.3 Statistics analysis The measured data were analyzed by SPSS 21 software. All counts were expressed using the rate. The prevalence of Helicobacter pylori infection in abdominal type of Henoch Schonlein purpura group, non abdominal type of Henoch Schonlein purpura group and healthy children group was compared with LineŚlist data X2 test. The treatment group and the routine group compared with the four grid data of Fisher exact probability method,P value of less than 0.05 was considered as statistical significance.Abdominal type of Henoch Schonlein purpura group and non abdominal type of Henoch Schonlein purpura group before treatment, measured serum C reactive protein for skewness distribution data, with a median and four percentile interval, between the two groups were compared using the Wilcoxon rank sum test,P value of less than 0.05 was considered as statistical significance. Before and after treatment in the treatment group and the routine group measured serum C reactive protein for skewness distribution data, with a median and four percentile interval, between the two groups were compared using the Wilcoxon rank sum test,P value of less than 0.05 was considered as statistical significance.Result: 1 Comparison of urea 13 C breath test Abdominal type of Henoch Schonlein purpura group measured Helicobacter pylori positive in 25 cases,15 cases of Helicobacter pylori negative patients;5 cases of non abdominal type of Henoch Schonlein purpura group measured Helicobacter pylori positive patients,35 cases of Helicobacter pylori negative patients;the hospital at the same time healthy children group measured Helicobacter pylori positive in 4 cases,28 cases of Helicobacter pylori negative;used by LineŚlist data X2 test, whether the difference among the three groups were compared with statistical significance;2 Comparison of therapeutic effects between the treatment group and the routine group The treatment group 10 cases were cured,1 case improved,2 cases ineffective;the routine group 1 case were cured,1 case improved,10 cases ineffective;using four grid data of Fisher exact probability method,P = 0.001(bilateral),shows the difference between the two groups was statistically significant(P< 0.05);3 Comparison of serum C reactive protein pretherapy with abdominal type of Henoch Schonlein purpura group and non abdominal type of Henoch Schonlein purpura group Abdominal type of group was 9.055(19.3)mg/L,non abdominal type of group was 1.4(5.2)mg/L,the comparison between the two groups using Wilcoxon rank sum test,Z=-5.043,P=0.000,indicating that the difference between the two groups was statistically significant(P< 0.05);4 Comparison of serum C reactive protein before the treatment in the treatment group and the routine group The treatment group was 19.9(51.9)mg/L,the routine group was 18.9(19.6)mg/L,the comparison between the two groups using Wilcoxon rank sum test,Z=-0.54,P=0.586,indicating that the difference between the two groups was not statistically significant(P>0.05);5 Comparison of serum C reactive protein after the treatment in the treatment group and the routine group. The treatment group was 3.7(2.5)mg/L,the routine group was 12(15.4)mg/L,and the two groups were compared with Wilcoxon rank sum test,Z=-2.614,P=0.009,indicating that the difference between the two groups was statistically significant(P<0.05);Conclusion: 1 The incidence of abdominal type of Henoch-Schonlein Purpura in children is related to the infection of Helicobacter pylori.2 Treatment of children with Helicobacter pylori infection of abdominal type of Henoch-Schonlein Purpura combined with anti Helicobacter pylori drugs can advance the curative effect, alleviate the clinical symptoms.3 CRP can be used to monitor the condition of abdominal type of Henoch-Schonlein Purpura in children.
