Objective:To investigate the clinical value of FeNO detection as an early judge whether children infected by RSV in lower respiratory tract will have recurrent coughs and asthma.Methods:A total of 852 cases,confirmed to be infected by respiratory syncytial virus in m? ltiple pathogen tests,who were receiving treatment for Frespiratory tract infection in hospital,were investigated from September 2009 to April 2016.Of these,426 cases were tested for FeNO,including 210 children with Sole-RSV infection.The 426 children were divided into six groups according to the res? lts of respiratory pathogen: Sole-RSV infection group,RSV combined bacterial infection group,RSV combined with mycoplasma infection group,RSV combined with virus infection group,RSV fungal infection group and RSV combined with m? ltiple pathogen infection group.The FeNO test res? lts as well as clinical data are compared between groups.Res? lts:The positive rate of FeNO detection and the number of FeNO in Sole-RSV infection group,RSV combined with bacterial infection group,RSV combined with mycoplasma infection group,RSV combined virus infection group and RSV combined with m? ltiple pathogen infection group,were compared.The positive rate of FeNO in RSV combined with m? ltiple pathogen infection group was the highest,16.7%.But there was no statistically significant difference in the positive rate of FeNO detection among the five groups.The numerical values of FeNO detection in children tested positive were statistically analyzed and compared.The numerical value of FeNO was 57.83 ±37.80 ppb in RSV combined with mycoplasma infection group,which was significantly higher than that in Sole-RSV infection group,which was 25.35±11.58 ppb.This difference in numerical value was statistically significant(P = 0.042);followed by RSV combined with m? ltiple pathogen infection group,whose numerical value of FeNO was 37.00 ± 12.40 ppb,which was significantly higher than that of Sole-RSV infection group(25.35±11.58 ppb)and that of RSV combined with virus infection group(P = 0.007;P = 0.048).There was no significant difference between remaining groups(P> 0.05).Through the analysis and comparison of the effect of a single factor on the numerical value of FeNO in the 210 cases of Sole-RSV infection group,it is found that the average age in the group with increased FeNO value was 0.63±0.89 years old,which was significantly higher than that of the group with normal FeNo value;children with eczema in the higher FeNO value group accounted for 54.2%,which was significantly higher than that of the normal FeNO value group,29%.Children tested positive in the allergen detection accounted for 45.8% in the higher FeNO value group,which was significantly higher than that of normal FeNO value group,5.9%.There were statistically significant differences.There is a higher proportion of patients suffering from allergic diseases in the family of children in the higher FeNO value group.But no significant differences existed in the group with normal FeNO values.The level of IgA serum of children in higher FeNO value group was significantly higher than the group with normal FeNo value in the laboratory examination,and the difference was statistically significant.As for the influence of m?ltiple factors on the FeNO detection value in Sole-RSV infection group,regression analysis shows that only eczema and allergen positive had independent influence on FeNO detection value.Conclusion:In the group infected by RSV and mycoplasma and the group infected by RSV and m? ltiple pathogens,FeNO detection value was significantly higher;children with eczema,allergen positive,allergic diseases in the family and increased serum IgA level,when suffering from lower respiratory tract infection,tend to have significantly increased RSV FeNO detection value.Children with pathogens infection,mycoplasma infection,eczema,allergic diseases in the family and increased serum IgA level,when infected by RSV in the lower respiratory tract,tend to have a high risk of developing into recurrent wheezing and asthma.
