Objectiv:To present a modified posteromedial approach for treatment of posterior pilon fracture with double-plating fixation.Methods:Eighteen patients were identified with posterior pilon fracture.All fractures were operated via modified posteromedial approach with double-plating fixation.The clinical outcome was evaluated with American Orthopaedic Foot and Ankle Society ankle-hind foot score and visual analogue scale.Radiological images were evaluated using osteoarthritis-score.Results:All fractures healed within a mean period of 12.6 weeks without malalignment or articular step-off.Fifteen patients were followed up for a mean period of 14.7 months,and all achieved good ankle function.All the patients achieved fracture union,and the mean healing time was 12.6 weeks.The average score of American Orthopaedic Foot & Ankle Society and visual analogue scale at rest,motion and weight bearing walking was 82.47±4.99 and 0.33,0.94,1.61 respectively.Conclusion:Modified posteromedial approach provides an alternative surgical treatment for posterior pilon fractures,and the short-term outcome was good. |
