| Objective1.Observe the effect of smectite on wound hemostasis and healing.2.Evaluate the feasibility of clip-endoloop method via a single channel endoscope for colonic mucosal defects.Method1.Observe the effect of smectite on wound hemostasis Clotting time in vitro of smectite group was compared with hemostatic powder group,starch group and blank control group.Then Male Sprague Dawley rats were used for making hemorrhage model.The rats with smectite powder were defined as the experimental group.The rats with commercial hemostatic powder were defined as the positive control group.The rats with starch were defined as the negative control group.The rats with nothing were defined as the blank control group.Each group had 8 rats.Hemostasis time and blood loss were measured respectively.One week later,rats were sacrificed and histopathology examination of the healing tissues were performed.2.Observe the effect of smectite on wound healing Male Sprague Dawley rats were used for making trauma model.On the rat dorsum,a skin defect was excised.The rats with smectite powder were defined as the experimental group.The rats with starch were defined as the negative control group.The rats with nothing were defined as the blank control group.Each group had 16 rats.On 2,4,6,8,10,12,14 days after operation,each group used the corresponding drug and the healing condition was recorded.Histopathology examination and sirus red stain of the healing tissues were performed to observe the tissue morphology.CD31 immunohistochemical staining was performed to observe the new capillaries.Hydroxyproline of the healing tissue was measured to indicate the tissue repair.3.Evaluate the feasibility of purse-string sutures with a single-channel colonoscope.We retrospectively analyzed 18 patients who had colonic post-ESD defects at our institute between June 2015 and June 2016,as a result of lesions excision,were treated using the single-channel endoscope technique.The results such as success rates,suture time,post-operational hospitalization,complications and total cost were evaluated in all patients.Results1.Observe the effect of smectite on wound hemostasisClotting time in vitro,hemostasis time and blood loss of smectite group were less than all control groups(p<0.05).The histopathology examination showed the wound healing process in smectite group was well.In addition,smectite had no evident damage on liver tissue.2.Observe the effect of smectite on wound healingOn 4,6,8,10,12,14 days after operation,the wound healing rate in smectite group was higher than control groups(p<0.05).The histopathology examination showed the wound healing process in smectite group was well and sirus red stain indicated the collagen fiber content in smectite group was more than control groups.Hydroxyproline measurement indicated the collagen content in smectite group was more than all control groups(p<0.05).CD31 immunohistochemical staining showed the smectite group had more new capillaries(p<0.05).3.Evaluate the feasibility of purse-string sutures with a single-channel colonoscope.All mucosal defects were successfully closed using the novel closure technique.The procedure commonly took 13.5±2.6 mins.The median post-operational hospitalization duration was 4.0 ±0.9 days.No severe complications were recorded.The mean total cost for hospital treatment was ¥28453.9±2983.7.The median follow-up period was 8.0±2.4 months.ConclusionThe mineral smectite powder promotes huge effect on wound healing:it can induce fast hemostasis and lower wound healing time.On the other hand,this novel clip-endoloop strategy via a single-channel endoscope is safe and feasible for endoscopic purse-string suture,which is worth being generated for clinic.Endoscopic spraying smectite maybe a simpler and more effective method to replace endoscopic purse-string sutures for post-operative wound healing in the future. |
