Objective To evaluate the efficacy and safety of dynamic pedicle screw fixation and interbody fusion in the treatment of lumbar degenerative diseases.Method Review of lumbar analysis from December 2014 to December 2016 in our hospital for surgical treatment of lumbar disc herniation and degenerative lumbar spine in 52 patients,were randomly divided into observation group and control group,the observation group(group A)to limit the use of dynamic pedicle screw in 23 cases of non fusion fixation,the control group(B group)using pedicle nail and 29 cases of patients with intervertebral bone fusion fixation.Observation and treatment efficacy and complications of the two groups were compared,such as operative time,bleeding volume,postoperative ambulation time,preoperative and postoperative pain score at different time points(VAS)and disability index(ODI),before and after the operation before the next and last follow-up near the the segment and the fixed displacement of vertebral and intervertebral height changes,postoperative lumbar flexion and extension of the change.Result Two groups of patients with a case of incision infection(all in the week after the incision along the original incision into the wound healing),no screw loosening and broken nail cases.Compared with the control group,the operation time,blood loss and postoperative time were significantly better than those in the control group,the difference was statistically significant(P<0.05).Two groups of patients were followed up,the VAS scores of the patients in the two groups were significantly improved compared with preoperative,the difference was statistically significant(P<0.05),and the effect of the observation group was significantly better than that of the control group.After 3-24 months follow-up,patients in the observation group compared with control group,lumbar activity range,the difference was statistically significant(P<0.05);both were compared between the two groups of fixed segmental vertebral height with the preoperative,the difference was not statistically significant(P>0.05);in the observation group near the upper section section of vertebral height compared with preoperative reduction is not obvious,the control group decreased significantly,the difference was statistically significant(P<0.05).Conclusion Lumbar restrictive dynamic pedicle screws compared interbody fusion and fixation with limited dynamic fixation has shorter operation time,less trauma,faster postoperative recovery,better effect,lumbar activity recover better,and in the case of reduction and prevention of adjacent segment degeneration has obvious advantages,provides a treatment plan for clinical.
